摘要
目的 探讨西那卡塞对腹膜透析患者皮肤瘙痒的疗效。方法 选取2020年1月-2022年1月我院收治的30例腹膜透析皮肤瘙痒患者,依据治疗方式差异将其分为西那卡塞组和加巴喷丁组,每组各15例。西那卡塞组口服西那卡塞片,加巴喷丁组口服加巴喷丁胶囊,比较两组瘙痒程度评分、甲状旁腺素(PTH)、血钙(Ca)、血磷(P)水平,通过Spearman系数分析腹膜透析患者皮肤瘙痒程度与PTH、Ca、P水平的相关性,统计两组不良反应发生率。结果 两组治疗前、治疗6个月后瘙痒程度评分、PTH、Ca、P水平组间比较,差异不具有显著性(P>0.05);与加巴喷丁组比较,西那卡塞组治疗1、3个月后瘙痒程度评分、PTH、P显著降低,Ca显著增高(P<0.05)。经相关性分析,瘙痒程度评分与PTH、P水平呈明显的正相关,与Ca呈明显的负相关(P<0.05)。两组仅出现乏力、恶心呕吐等轻微不良反应,未经特殊处理均自行缓解。结论 西那卡塞治疗腹膜透析皮肤瘙痒患者通过改善甲状旁腺素及钙磷代谢水平可以缓解瘙痒症状。
Objective: To explore the therapeutic effect of Sinacase on pruritus in peritoneal dialysis patients.Methods: 30 patients with pruritus in the treatment of peritoneal dialysis in our hospital from January 2020 to January2022 were selected, and were divided into Sinacase group and gabapentin group according to the difference of treatment methods, with 15 cases in each group. Sinacase group took Sinacase tablets orally, and Gabapentin group took Gabapentin capsules orally. The pruritus score, parathyroid hormone(PTH), blood calcium(Ca) and blood phosphorus(P) levels were compared between the two groups. The correlation between pruritus and PTH, Ca and P levels in peritoneal dialysis patients was analyzed by Spearman coefficient, and the incidence of adverse reactions was counted between the two groups. Results: There was no significant difference in pruritus score, PTH, Ca and P levels between the two groups before and after 6 months of treatment(P>0.05). Compared with gabapentin group, the pruritus score,PTH, P and Ca in Sinacase group decreased significantly after 1 and 3 months of treatment(P<0.05). According to the correlation analysis, the pruritus score was positively correlated with PTH and P levels, and negatively correlated with Ca(P<0.05). Only slight adverse reactions, such as fatigue, nausea and vomiting, occurred in the two groups, which were all relieved by themselves without special treatment. Conclusion: Sinacase can relieve pruritus by improving the metabolism levels of parathyroid hormone and calcium and phosphorus in patients with pruritus during peritoneal dialysis.
