摘要
目的了解成都市青年学生男男性行为者(MSM)非固定性伴性行为的特征以及与社会规范的关系,为艾滋病相关风险行为的干预措施提供依据。方法在成都市6个行政区招募青年学生MSM调查员,通过滚雪球和受试者推荐混合抽样方法,对调查员社交圈中的青年学生MSM进行性行为特征和社会规范的调查。结果共调查221人,平均年龄(21.16±2.58)岁,研究生及以上文化程度26人(11.76%);性取向为同性恋197人(89.14%);近6个月有非固定性伴性行为58人(26.24%),其中性伴数量≥2个的有35人(60.34%);未坚持每次性行为都使用安全套11人(18.97%)。多因素分析发现,首次肛交性行为对象为非恋爱对象是最近6个月非固定性伴性行为的危险因素[比值比(OR)=5.585,95%可信区间(CI)=2.466~12.650],非固定性伴性行为社会规范对性行为有影响,不认同"寻找非固定性伴可以接受"(OR=0.178,95%CI=0.043~0.748)是最近6个月发生非固定性伴性行为的保护因素。结论需要进一步关注青年学生MSM社群亚文化及社会规范对其行为的影响,可将同志社交软件作为青年学生MSM的重要性安全知识宣传与干预平台,促进性伴之间防艾知识的交流与利用,培养健康的恋爱观并加强性安全责任规范,减少艾滋病传播风险。
Objective To understand the characteristics of sex behaviors of young MSM(men who have sex with men) students with casual partners in Chengdu and their relationship with social norms, so as to provide basis for intervention measures of AIDS-related risk behaviors. Methods Young MSM students were recruited as investigators in 6 administrative districts of Chengdu, and their sexual behavior characteristics and social norms were investigated in their social circle through snowballing and mixed sampling methods. Results Among the 221 investigated, the average age was 21.16 ± 2.58 years. 26(11.76%) were graduate students or with higher education. 197(89.14%) was homosexual. In the past 6 months, 58(26.24%) had intercourse with casual partners, among which 35(60.34%) had intercourse with no less than 2 partners. 11(18.97%) did not use condom consistently every time of sex. Multivariable analysis found that the first time anal sex with casual partners was the risk factor(OR = 5.585, 95% CI = 2.466-12.650) of sexual behaviors in recent 6 months, which also had impact on their social norms. The protective factor was that "it is unacceptable to seek casual partners"(OR =0.178, 95% CI =0.043-0.748) in recent 6 months. Conclusion Further attention should be attached to MSM community sub-culture and the impact of social norms on the young MSM’ sexual behavior. Gay dating app can be used as a platform to publicize the knowledge of safe sex behavior and to promote the communication and utilization of AIDS prevention knowledge among sexual partners so as to reduce the risk of HIV transmission.
