摘要
目的探讨改良剥脱术治疗大隐静脉曲张合并浅静脉血栓的疗效。方法选择2015年1月至2018年3月武警新疆总队医院收治的120例大隐静脉曲张合并浅静脉血栓患者,按照治疗方法不同分为对照组和观察组,各60例。对照组施行高位结扎联合分段切除术,观察组施行改良高位结扎联合点状剥脱术,比较两组的临床疗效、症状评分、手术情况、术后并发症发生率、术后复发率。结果治疗后,两组临床总有效率、症状评分比较,差异无统计学意义;两组下肢肿胀、色素沉着、下肢溃烂和下肢酸痛评分均低于治疗前,差异有统计学意义(P<0.05);观察组手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组术后并发症总发生率3.33%低于对照组15.00%,差异有统计学意义(P<0.05)。术后随访1年,观察组复发率为1.67%低于对照组复发率为13.33%,差异有统计学意义(P<0.05)。结论改良高位结扎联合点状剥脱术治疗大隐静脉曲张合并浅静脉血栓的临床疗效显著,具有手术创伤小、术后并发症少、复发率低等优势。
Objective To investigate the effect of modified exfoliation in the treatment of varicose great saphenous vein with superficial venous thrombosis.Method 120 patients with varicose great saphenous vein complicated with superficial venous thrombosis were selected from January 2015 to March 2018 in Xinjiang General Hospital of Armed Police.They were divided into control group and observation group according to different treatment methods,60 cases in each group.The control group was treated with high ligation combined with segmental resection,while the observation group was treated with modified high ligation combined with punctate stripping.The clinical efficacy,symptom score,operation condition,incidence of complications and recurrence rate were compared between the two groups.Result After treatment,there was no significant difference in the total effective rate and symptom score between the two groups.The scores of lower limb swelling,pigmentation,ulceration and soreness in the two groups were lower than those before treatment,and the difference was statistically significant(P<0.05);the operation time of the observation group was shorter than that of the control group,and the amount of bleeding during the operation was less than that of the control group,with statistical significance(P<0.05).The total incidence of complications in the observation group was 3.33%lower than that in the control group 15.00%,with statistical significance(P<0.05).After one year follow-up,the recurrence rate in the observation group was 1.67%,which was lower than that in the control group 13.33%,with statistical significance(P<0.05).Conclusion Modified high ligation combined with punctate stripping is effective in the treatment of varicose great saphenous vein with superficial venous thrombosis.It has the advantages of less trauma,fewer complications and lower recurrence rate.
