摘要
胃癌是典型的消化道恶性肿瘤,病理特征方面以远处转移与腹膜种植最为突出,腹腔内游离癌细胞是腹膜转移的主要原因。因此,腹腔内游离癌细胞清除是局部进展期胃癌治疗的重要目标。近年,腹腔热灌注化疗(hyperthermic intraperitoneal chemotherapy,HIPEC)由于其有效的治疗效果以及可靠的操作性得到医疗界广泛认可与普及,其在进展期胃癌治疗中也发挥着关键作用。全文主要就HIPEC的起源发展、理论基础、应用标准、治疗流程,以及临床应用实况等进行分析。
Peritoneal implantation is common in gastric cancer,and the free cancer cells in the abdominal cavity are the main cause of peritoneal metastasis.Therefore,intraperitoneal free cancer cell clearance has become an important treatment for locally advanced gastric cancer.In recent years,hyperthermic intraperitoneal chemotherapy(HIPEC)has been widely used due to its effective therapeutic effect and reliable operability,and also plays a key role in the treatment of advanced gastric cancer.This article reviews the origin and development,theoretical basis,application standards,treatment process,and clinical application of HIPEC.
