摘要
目的探讨哑铃负重摆臂运动在血透患者动静脉内瘘成熟中的应用。方法选取2018年2月至2020年2月我院88例血透患者动静脉内瘘患者为研究对象,随机分为对照组(n=44例)和观察组(n=44例)。对照组予以常规握球锻炼,观察组在此基础上采用哑铃负重摆臂运动,比较两组患者动静脉内瘘血管血流动力学指标、两组干预依从度情况。结果两组患者自体动静脉内瘘的引流静脉(draining vein,DV)血流量,两组间差异有统计学意义(F=20.487,P<0.001),不同时间点间的差异有统计学意义(P<0.05)。观察组干预后出现血栓、感染、血管瘤的动静脉内瘘并发症发生率低于对照组(4.54%vs25.00%,P<0.05)。观察组患者干预总依从度97.73%高于对照组86.36%(P<0.05)。结论针对血透患者动静脉内瘘的临床干预中,采用早期哑铃负重摆臂运动相比常规握球锻炼可充分利用康复资源,减少并发症发生,全面提升患者治疗依从性,并且利于动静脉内瘘成熟,值得推广和应用。
Objective To investigate the application of dumbbell weight swinging arm movement in hemodialysis patients with arterio-venous fistula maturation.Methods A total of 88 hemodialysis patients with arteriovenous internal fistula treated in our hospital from February 2018 to February 2020 were selected as the research objects,and randomly divided into control group(n=44 cases)and observation group(n=44 cases).The control group was given routine ball-holding exercise,and the observation group was given dumbbell weight-bearing arm swing exercise on this basis.Hemodynamic indexes of arteriovenous fistula and intervention compliance of the two groups were compared.Results The blood flow amount of draining vein(DV)of autologous arteriovenous fistula was statistically significant between the two groups(F=20.487,P<0.001),and the difference was statistically significant between the two groups at different time points(P<0.05).The incidence of arteriovenous fistula complications with thrombosis,infection and hemangioma in the observation group was lower than that in the control group(4.54%vs 25.00%,P<0.05).The total compliance of the observation group was 97.73%,which was higher than 86.36%of the control group(P<0.05).Conclusion In the clinical intervention of arteriovenous fistula in hemodialysis patients,the use of early dumbbell bearing swing arm exercise compared with conventional ball-holding exercise can make full use of rehabilitation resources,reduce complications,comprehensively improve the treatment compliance of patients,and is conducive to the maturity of arteriovenous fistula,which is worthy of promotion and application.
