摘要
目的探究不同检测结核分枝杆菌方法比较及应用价值。方法选取2019年1月-2020年6月本院收治的疑似肺结核患者痰标本425例。分别采用荧光二极管直接涂片法、酸性罗氏培养法、Bactec MGIT 960培养法,以临床诊断作为金标准,分析三种方法诊断效能,并分析三种方法联合检测的诊断效能。结果临床诊断肺结核247例,非肺结核178例,LED直接涂片法检测灵敏度、一致率分别为(22.27%、0.160),酸性罗氏培养检测灵敏度、一致率分别为(31.98%、0.254),Bactec MGIT 960培养检测灵敏度、一致率分别为(43.62%、0.390),Bactec MGIT 960培养敏感度和一致率最高,其次为酸性罗氏培养、LED直接涂片法。三种方式联用检测敏感度、一致率(49.80%、0.401)高于LED直接涂片法与酸性罗氏培养联用(34.8%、0.276)、酸性罗氏培养与Bactec MGIT 960培养联用(43.72%、0.361)。结论Bactec MGIT 960培养法检测灵敏度最高,其次为酸性罗氏培养、LED直接涂片法,而三种方式联用检测可提高检测敏感度及与临床诊断一致率。
Objective To explore the comparison and application value of different methods for detecting Mycobacterium tuberculosis.Methods 425 sputum samples of suspected tuberculosis patients admitted to our hospital from February 2019 to June 2020were collected and tested through three methods including the fluorescent diode direct smear method,acid Roche culture method,and Bactec MGIT 960 culture method.Taking clinical diagnosis as the gold standard,the diagnostic efficacy of the three methods and the diagnostic efficacy of the combined detection of the three methods were analyzed.Results There were 247 cases of pulmonary tuberculosis and 178 cases of non-pulmonary tuberculosis.The sensitivity and consistency of LED direct smear method were 22.27%and0.160 respectively.The sensitivity and consistency of acid Roche culture were(31.98%and 0.254);the sensitivity and consistency of Bactec MGIT 960 culture were(43.62%and 0.390),respectively.Bactec MGIT 960 culture had the highest sensitivity and consistency,followed by acidic Roche culture and LED direct smear method.The detection sensitivity and consistency of the combined three methods(49.80%,0.401)were higher than that of the LED direct smear method combined with acid Roche culture(34.8%,0.276)and that of acid Roche culture combined with the Bactec MGIT 960 culture(43.72%,0.361).Conclusion Bactec MGIT 960 culture method has the highest detection sensitivity,followed by acidic Roche culture and LED direct smear method.The combined detection of the three methods can improve the sensitivity of detection and the consistency of clinical diagnosis.
