摘要
目的研究甲状腺癌分化转移与外周血相关微小核糖核(microRNA,miRNA)表达水平的相关性。方法从本院2018年-2021年间经病理诊断检确诊为乳头状甲状腺癌患者中选择48例转移的乳头状甲状腺癌患者作为研究组(乳头状甲状腺癌25例、甲状腺髓样癌5例、滤泡型甲状腺癌12例,未分化甲状腺癌6例),另选择未转移的48例乳头状甲状腺癌患者作为对照组(乳头状甲状腺癌25例、甲状腺髓样癌4例、滤泡型甲状腺癌13例,未分化甲状腺癌6例)。收集两组患者的血清,检测两组患者血清中的miRNA-221、miRNA-222、miRNA-146b、miRNA-21、miRNA125b、miRNA-181a的表达水平。收集两组患者的病变组织,应用免疫组织化学的方法检测N-cadherin、MMP9的表达水平。结果在本研究中,研究组患者血清中的mi RNA-221、miRNA-222表达水平显著高于对照组(P<0.05);研究组患者与对照组患者之间miRNA-146b、miRNA-21差异不显著(P>0.05);对照组患者血清中的miRNA-125b的表达水平显著高于研究组(P<0.05),研究组患者血清中的miRNA-181a表达水平显著低于对照组患者(P<0.05);研究组患者N-cadherin、MMP9阳性率显著高于对照组(P<0.05)。结论在判断甲状腺癌肿瘤的分化转移与预后中,应用检测血清中miRNA水平的方法具有较高的应用价值,为医生确定治疗方案、做出诊断提供了有效的信息。
Objective To investigate the correlation between the differentiation and metastasis of thyroid cancer and the expression level of micro RNA(mi RNA).Methods 48 patientswho were diagnosed with transferred papillary thyroid carcinoma in our hospital from 2018 to 2021 were chosen as the research group including 25 cases with papillary thyroid carcinoma,5 cases with medullary thyroid carcinoma,12 cases with follicular thyroid carcinoma and 6 caseswith undifferentiated thyroid carcinoma.48 patients with unmetastatic papillary thyroid carcinoma were selected as the control group including 25 cases of papillary thyroid carcinoma,4 cases of medullary thyroid carcinoma,13 cases of follicular thyroid carcinoma and 6 undifferentiated thyroid carcinoma.Serum from both groups were collected and the expression levels of mi RNA-221,mi RNA-222,mi RNA-146b,mi RNA-21,mi RNA125b,and mi RNA-181a were measured.Lesional tissues were collected from both groups,and the expression levels of N-cadherin and MMP9were determined by immunohistochemistry method.Results In this study,miRNA-221 and miRNA-222 expression levels in the research group were significantly higher than that of the control group(P<0.05).miRNA-146b and miRNA-21 had no significant differences between two groups(P>0.05).miRNA-125 bin control group was significantly higher than that in study group(P<0.05).miRNA-181a in study group was significantly lower than that of control patients(P<0.05).The positive rate of N-cadherin and MMP9was significantly higher than that of control group(P<0.05).Conclusion Detection of serum mi RNA levels has a high application value in determing the differentiation,metastasis and prognosis of thyroid cancer,which provides effective information for doctors to determine the treatment plan and make diagnosis.
