摘要
目的了解新疆维吾尔自治区重症监护病房(ICU)连续性肾脏替代治疗(CRRT)应用现况,针对目前CRRT应用存在的问题进行分析,为自治区CRRT的应用及发展提供理论基础。方法采用网络调查表的形式对疆内30家质控机构进行横断面调查。结果参与调查的质控对象,临床医生开具实行CRRT治疗处方到患者上机的平均时间为2.8 h,单次治疗持续时间多为24 h以上。抗凝方式多采用枸橼酸抗凝,占比为55.1%,前稀释为最常见的置换液稀释方式,占52.2%。可全流程开展CRRT医师150名,参培医师为71名,占医师总数的30.34%(71/234),可全流程开展CRRT护士共198名,而参培护士共98名,占护士总数的12.52%(98/783)。CRRT收费标准平均为126.7元/h,总费用为16746.52,占ICU花费的38.3%。结论我省目前已广泛开展CRRT治疗,但CRRT费用成本仍旧较高,未来仍需要多方面努力以降低患者治疗花费。
Objective To investigate the application status of continuous renal replacement therapy(CRRT)in intensive care units(ICU)in Xinjiang Uygur Autonomous Region,and to analyze the existing problems in the application of CRRT,so as to provide a theoretical basis for the application and development of CRRT.Methods A cross-sectional survey was conducted in 35 ICU of 30 quality control medical institutions in Xinjiang Uygur Autonomous Region.Results Among the quality control subjects in the survey,the average time from the clinician's prescription for CRRT treatment to beginningtimeoftreatment of the patients was 2.8h,and the duration of single treatment was more than 24h.Citric acid was most commonly used in anticoagulation,accounting for 55.1%,and pre-dilution was the most common replacement solution dilution,accounting for 52.2%.150 physicians were able to carry out CRRT in the whole process,and 71 cases beingparticipated in training were able to carry out CRRT in the whole process,accounting for 30.34%(71/234)of the total number of physicians.A total of 198 nurses were able to carry out CRRT in the whole process,while 98 participated in training were able to carry out CRRT in the whole process,accounting for 12.52%(98/783)of the total number of nurses.The average charge standard of CRRT was 126.7 yuan/hour,and the average total CRRT cost was 16,746.52 yuan,accounting for 38.3%of the ICU cost.Conclusion CRRT treatment has been widely carried out in our province,butthe cost of CRRT is still high,and efforts in many aspects are still needed to reduce the treatment cost of patients in the future.
