摘要
目的探究老年糖尿病合并非酒精性脂肪性肝病(NAFLD)患者肝纤维化程度与尿微量白蛋白的相关性。方法选取本院2020年3月1日-2021年3月1日收治的老年糖尿病合并NAFLD患者的76例患者作为研究组,选取同期住院的单纯糖尿病患者76例作为对照组,评估患者NAFLD纤维化评分(NFS),采用Pearson相关分析法分析老年糖尿病合并NAFLD患者肝纤维化程度与微量白蛋白尿之间的相关性。结果(1)24 h尿蛋白定量:两组不同时间点尿微量白蛋白比较差异有统计学意义,研究组不同时间尿微量白蛋白均高于对照组,存在组间及时间效应(P<0.05);(2)NFS:两组不同时间点NFS评分比较差异有统计学意义,研究组不同时间NFS评分均高于对照组,存在组间及时间效应(P<0.05);(3)相关性分析结果显示:老年糖尿病合并NAFLD患者肝纤维化程度与24 h白蛋白尿呈现正相关(r=0.477,P<0.05)。结论肝纤维化程度与微量白蛋白尿参与老年糖尿病合并NAFLD患者疾病发展,在治疗过程中应注意警惕肝纤维化风险和蛋白尿的发生。
Objective To investigate the correlation between the degree of liver fibrosis and microalbuminuria in elderly diabetic patients with nonalcoholic fatty liver disease(NAFLD).Methods 76 elderly diabetic patients with NAFLD admitted in our hospital from March 1,2020 to March 1,2021 were selected as the study group,and 76 patients with simple diabetes hospitalized during the same period were selected as the control group.The NAFLD fibrosis score(NFS)of patients was evaluated.Pearson correlation analysis was used to analyze the correlation between the degree of liver fibrosis and microalbuminuria in elderly diabetic patients with NAFLD.Results(1)24h urinary protein quantification:There was a statistically significant difference between the two groups in terms of urinary microalbumin at different time points.The urinary microalbumin in the study group was higher than that in the control group at different time,with inter-group and time effects(P<0.05).(2)NFS:The NFS scores of the two groups at different time points were statistically significant.The NFS scores of the study group at different time points were higher than those of the control group,and there were inter-group and time effects(P<0.05).(3)Correlation analysis showed that the degree of liver fibrosis in elderly diabetic patients with NAFLD was positively correlated with 24h albuminuria(r=0.477,P<0.05).Conclusion The degree of liver fibrosis and microalbuminuria are involved in the development of disease in elderly patients with diabetes mellitus combined with NAFLD,and attention should be paid to the risk of liver fibrosis and the occurrence of proteinuria during treatment.
