摘要
目的观察增强MRI联合血清相关指标对肝癌微血管侵犯(MVI)的预测价值。方法回顾性分析,收集2020年1月-2021年12月本院收治的79例肝癌患者临床资料,查阅资料,根据手术病理结果,记录患者MVI确诊情况,并分为MVI组、非MVI组;患者入院时均接受增强MRI检查及血清标志物检测,比较两组一般资料及增强MRI检查参数、血清甲胎蛋白(AFP)水平;分析上述指标与肝癌MVI的关系,进一步探究增强MRI联合血清指标对肝癌MVI的预测价值。结果79例肝癌患者中,经查阅资料发现,有30例患者存在MVI,占37.97%;MVI组动脉期熵低于非MVI组,灰度共生矩阵群显著性、灰阶非均匀性、血清AFP均高于非MVI组,有统计学差异(P<0.05);两组门静脉期灰度共生矩阵差分熵比,无统计学差异(P>0.05);经Logistic回归分析,结果显示,熵、灰度共生矩阵群显著性、灰阶非均匀性、血清AFP与肝癌MVI发生有关(P<0.05);绘制ROC曲线图,结果显示,增强MRI中熵、灰度共生矩阵群显著性、灰阶非均匀性及血清AFP单独及联合检测对肝癌MVI均有一定预测价值,其联合检测预测价值更高。结论增强MRI相关参数、血清AFP与肝癌MVI的发生有关,联合检测可提高肝癌MVI的预测价值,为治疗方案拟定提供参考。
Objective To observe the predictive value of contrast-enhanced MRI combined with serum-related indexes in microvascular invasion(MVI)of liver cancer.Methods Retrospective analysis was performed,the clinical data of 79 patients with liver cancer who were admitted to our hospital from January 2020 to December 2021 were collected.After consulting the data,the confirmed MVI status of the patients was recorded according to the surgical pathological results,and they were divided intoMVI group and non-MVI group.All patients underwent enhanced MRI examination and serum marker detection when they were admitted to hospital.The general data,enhanced MRI examination parameters,and serum alpha-fetoprotein(AFP)levels were compared between the two groups;the relationship between the above indicators and liver cancer MVI was analyzed,and the combination of enhanced MRI and serum markers was further explored.Then the value of enhanced MRI combined with serum indicators in predicting MVI of liver cancer was further explored.Results Among the 79 patients with liver cancer,it was found that 30 patients had MVI,accounting for 37.97%.The arterial phase entropy in the MVI group was lower than that in the non-MVI group,and the significance of gray co-occurrence matrix group,non-uniformity of gray scale and serum AFP were higher than those of non-MVI group,with statistical differences(P<0.05);the difference entropy ratio of the gray co-occurrence matrix of the portal venous phase between the two groups had no statistical difference(P>0.05).Logistic regression analysis showed that entropy,gray co-occurrence matrix group significance,gray scale heterogeneity and serum AFP were related to the occurrence of MVI(P<0.05).ROC curve was drawn,and the results showed that the entropy,gray co-occurrence matrix group significance,gray-scale heterogeneity,and serum AFP detection alone and combined have certain predictive value for liver cancer MVI,and the combined detection has higher predictive value.Conclusion Enhanced MRI-related parameters and serum AFP are related to the occurrence of MVI in liver cancer.Combined detection can improve the predictive value of MVI in liver cancer and provide a reference for the formulation of treatment plans.
