超声造影联合心肌应变测值评估兔子急性心肌梗死 被引量:2

The evaluation of contrast-enhanced echocardiography combined with myocardial strain measurements for acute myocardial infarction in rabbits

摘要 目的探讨超声造影联合心肌应变测值评估兔子急性心肌梗死的影像学特点。方法选取健康成年兔20只,随机分为观察组和对照组各10只,观察组建立急性心肌梗死模型,对照组仅在相应部位绕线不结扎。对两组兔行超声造影联合心肌应变测值及病理组织学检查,比较二维超声心动图参数[左心室收缩末期内径(LVESd)、左心室舒张末期内径(LVEDd)、左心室短轴缩短率(LVFS)、左心室射血分数(LVEF)、室壁节段运动积分指数评分(WMSI)],评估手术前后观察组左心室长轴3个切面(二腔心、三腔心、四腔心)的心脏纵向应变以及两组左心室短轴乳头肌水平6个节段(前间隔、前壁、前侧壁、后侧壁、后间隔、后壁)的心脏圆周向应变、心脏轴向应变值。另外,比较心肌超声造影切面与心肌梗死病理切片的梗死阶段数,计算两者的重合数量和符合率。结果观察组LVESd值、LVEDd值和WMSI评分均高于对照组,而LVFS值、LVEF值均低于对照组(t=9.676、2.671、7.378、7.648、7.195,P<0.05)。观察组术后与术前比较,左心室3个切面的纵向应变均降低。观察组前间隔、前壁、前侧壁、后侧壁、后间隔、后壁的圆周向应变值、轴向应变值均低于对照组,差异均有统计学意义(t=25.702、32.779、24.276、6.259、9.877、5.627、38.588、16.041、21.037、12.953、10.230、5.231,P<0.05)。与对照组比较,观察组心肌血流灌注降低。心肌超声造影切面与心肌梗死病理切片比较,两者梗死节段重合60个,符合率93.8%。结论超声造影联合心肌应变测值对评估兔子急性心肌梗死具有较高的符合率,值得临床应用。 Objective To explore the image feature of the acute myocardial infraction in rabbits by contrast-enhanced echocardiography combined with myocardial strain measurements.Methods Twenty healthy adult rabbits were randomly divided into two groups named observation group and control group,with 10 rabbits in each group.The model of acute myocardial infarction was established in the observation group,while the control group was wound only at the appropriate parts without ligation.Contrastenhanced echocardiography combined with myocardial strain measurements and histopathological examination were performed in the two groups,the two-dimensional echocardiography parameters[left ventricular end-systolic diameter(LVESd),left ventricular end-diastolic diameter(LVEDd),left ventricular fractional shortening(LVFS),left ventricular ejection fraction(LVEF),wall motion score index(WMSI)]were compared,the longitudinal strains of three sections of the long axis of left ventricle(twochamber heart,three-chamber heart,four-chamber heart)in the observation group and the circumferential strains and axial strains of six segments of the short axis of left ventricle papillary muscle(anterior septum,anterior wall,anterior lateral wall,posterior lateral wall,posterior septum,posterior wall)in the two groups were evaluated before and after surgery.In addition,the numbers of infarct stages detected by myocardial contrast-enhanced echocardiography and pathological sections of myocardial infarction were compared,and the coincidence number and coincidence rate were calculated.Results LVESd,LVEDd and the scores of WMSI in the observation group were higher than those in the control group,while LVLVFS and LVEF were lower than those in the control group(t=9.676,2.671,7.378,7.648,7.195;P<0.05).The longitudinal strain measurements of three sections of left ventricle in the observation group decreased after operation compared with those before operation.The circumferential strain and axial strain measurements of the anterior septum,anterior wall,anterior lateral wall,posterior lateral wall,posterior septum and posterior wall in the observation group were lower than those in the control group,and the differences were statistically significant(t=25.702,32.779,24.276,6.259,9.877,5.627,38.588,16.041,21.037,12.953,10.230,5.231;P<0.05).Compared with the control group,myocardial blood perfusion decreased in the observation group.The results of comparison of myocardial contrastenhanced ultrasound echocardiography and pathological sections of myocardial infarction showed that there were 60 overlapped infarct segments between the two groups,and the coincidence rate was 93.8%.Conclusion Contrast-enhanced echocardiography combined with myocardial strain measurements has a high coincidence rate in the evaluation of acute myocardial infarction in rabbits,which is worthy of clinical application.
