血清晚期糖基化终产物及肾小球滤过率交互作用对慢性肾衰竭患者颈动脉粥样硬化发生的影响 被引量:3

The effect of the interaction of serum advanced glycation end products and glomerular filtration rate on carotid atherosclerosis in patients with chronic renal failure

摘要 目的探讨血清晚期糖基化终产物(AGEs)和肾小球滤过率(eGFR)的交互作用及其对慢性肾衰竭(CRF)患者发生颈动脉粥样硬化(CAS)的影响。方法选取2018年3月至2020年4月在台州医院肾内科确诊为CRF的患者126例,依据超声检查结果将患者分为CAS组67例和NCAS组59例。比较两组患者的临床资料包括AGEs、eGFR等指标,通过Logistic回归分析CRF患者发生CAS的影响因素,分析影响因素间的交互作用,采用受试者工作特征(ROC)曲线预测影响因素对CRF患者发生CAS的诊断效能。结果高水平的AGEs和低水平的eGFR为CRF患者发生CAS的独立危险因素[OR(95%CI)=35.672(26.154~39.823)、8.703(5.634~11.031),P<0.05],并且两者存在正交互作用[OR(95%CI)=6.729(6.137~6.909),P<0.01],ROC曲线分析结果显示两者协同作用对CAS发生风险的诊断价值(AUC=0.852,95%CI=0.797~0.872,P<0.01)大于两者单独的诊断价值。结论AGEs和eGFR之间存在正交互作用,且两者协同作用对CRF患者发生CAS的风险有较好的诊断效能。 Objective To explore the interaction between serum advanced glycation end products(AGEs)and glomerular filtration rate(eGFR)and its effect on carotid atherosclerosis(CAS)in patients with chronic renal failure(CRF).Methods A total of 126 patients diagnosed with CRF in the department of nephrology,Taizhou Hospital from March 2018 to April 2020 were selected,and divided into CAS group(67 cases)and non-CAS(NCAS)group(59 cases)according to the ultrasound examination results.The clinical data including AGEs and eGFR were compared,and the influence factors of CAS in patients with CRF were analyzed by logistic regression analysis,the interaction between the influence factors were analyzed,and the receiver operating characteristic(ROC)curve were used to predict the diagnostic efficacy of influence factors for CAS in CRF patients.Results High levels of AGEs and low levels of eGFR were independent risk factors for CAS in CRF patients[OR(95%CI)=35.672(26.154-39.823),8.703(5.634-11.031);P<0.05],and there was a positive interaction between them[OR(95%CI)=6.729(6.137-6.909),P<0.01].ROC curve analysis showed that the diagnostic value of the synergistic effect of the two on the risk of CAS(AUC=0.852,95%CI=0.797-0.872,P<0.01)was greater than the diagnostic value of the two alone.Conclusion There is a positive interaction between AGEs and eGFR,and the synergistic effect of the two has a better diagnostic performance on the risk of CAS in CRF patients.
