摘要
目的:通过查阅中药治疗肛肠术后便秘的临床文献,对所涉及中药进行统计学分析,探究其用药规律,为现代临床精准用药和新药研发提供参考。方法:以"肛肠术后便秘""中药"等为关键词查阅并收集中国知网、万方、维普3个数据库1998年1月至2020年7月治疗该病的临床文献,利用数据挖掘技术对符合纳入标准的文献中药进行频数分析、关联规则分析、聚类分析。结果:共查找到符合标准的文献144篇,方剂157个,其中使用频次最高的6种药物分别是火麻仁、大黄、枳实、苦杏仁、厚朴、当归。药性统计分析,四气中寒性频次最高;五味中甘味最频;归经统计分析,胃经最频;药物类别主要为补虚药,关联规则分析得到10条常用组合,其中最常用的是白芍+苦杏仁→火麻仁。聚类分析得到5组常用分类。结论:中医治疗肛肠术后便秘以虚为主,虚实夹杂,治疗以健脾补肾、调畅气机、滋润肠道为主,辅以清热凉血、活血化瘀、利水渗湿。
Objective:By consulting the clinical literature about the treatment of constipation after anorectal surgery with traditional Chinese medicine,statistically analyzing the traditional Chinese medicines involved,and exploring rules of medicine use,this paper intends to provide reference for modern clinical precise medicine use and R&D of new medicines.Methods:With keywords such as"constipation after anorectal surgery"and"traditional Chinese medicine",the clinical literatures about the treatment of the disease from January 1998 to July 2020 in CNKI,Wanfang and VIP databases were consulted and collected.The frequency analysis,association rule analysis and cluster analysis were made on literatures of traditional Chinese medicine meeting the inclusion criteria by using the data mining technology.Results:A total of 144 literatures and 157 prescriptions meeting the criteria were found.Among them,the 6 medicines with the highest frequency of use were Semen Cannabis,rheum officinale,immature bitter orange,semen armeniacae amarae,Magnolia officinalis and Angelica sinensis.According to the statistical analysis of the property of medicine,cold property was used the most frequently in the four qi and sweet flavor was used the most frequently in the five flavors.According to the statistical analysis,stomach meridian was used the most frequently.The main type of medicine was deficiency tonifying medicine.10 commonly used combinations were obtained by association rule analysis,among which the most commonly used combination was radix paeoniae alba+semen armeniacae amarae→Semen Cannabis.Five groups of common classification were obtained by cluster analysis.Conclusion:In traditional Chinese medicine,constipation after anorectal surgery is mainly treated based on deficiency and excessiveness and deficiency are mixed.It is mainly treated by tonifying spleen and kidney,regulating qi activity and moistening the intestinal tract.It is supplemented by removing heat to cool blood,promoting blood circulation to remove blood stasis and clearing damp and promoting diuresis.
