摘要
目的:探讨术后即时T1WI评估肝脏恶性肿瘤热消融术疗效的临床价值。方法:收集2018年6月—2020年12月长海医院79例接受CT引导下射频消融或微波消融治疗的肝脏恶性肿瘤患者资料。79例均行术前2周磁共振增强扫描、术后即时T1WI平扫及术后1~3个月磁共振增强扫描。对比术后两次检查结果,论证术后即时T1WI评估效能。分析最小消融边界与热消融术疗效之间的关系。采用t检验或Fisher精确检验进行统计学分析。结果:术后即时T1WI评估完全消融率为100%,术后1~3个月MR增强检查评估完全消融率为93.67%,两者差异无统计学意义(P=0.069)。不完全消融组的最小消融边界[(0.132±0.138)cm]明显低于完全消融组的最小消融边界[(0.427±0.260)cm],差异有统计学意义(P=0.015)。结论:术后即时T1WI可以用于早期评估肝脏恶性肿瘤热消融术疗效,对于指导后续治疗方案的制订具有较显著的临床意义。
Purpose:To investigate the clinical value of immediate postoperative T1WI in evaluating the efficacy of CT-guided thermal ablation.Methods:Data of 79 patients with liver malignant tumor treated by CT-guided radiofrequency ablation or microwave ablation were collected from June 2018 to December 2020 in Changhai Hospital.All 79 patients underwent enhanced MRI 2 weeks before surgery,T1WI plain MRI and enhanced MRI 1-3 months after surgery.The results of two postoperative examinations were compared to demonstrate the efficacy of immediate postoperative T1WI evaluation.The relationship between minimum ablation boundary and thermal ablation effect was analyzed.Statistical analysis was performed using t-test or Fisher’s exact test.Results:The complete ablation rate assessed by T1WI immediately after surgery and by MR enhancement examination 1-3 months after surgery was 100%and 93.67%,respectively,and the difference between them was with no statistical significance(P=0.069).The minimal ablation margin of incomplete ablation group[(0.132±0.138)cm]was significantly lower than that of complete ablation group[(0.427±0.260)cm],and the difference was with statistical significance(P=0.015).Conclusion:Immediate postoperative T1WI can be used for early evaluation of the efficacy of CT-guided thermal ablation,which has significant clinical significance in guiding the development of follow-up treatment plan.
