白细胞介素-1受体相关激酶-M在慢性阻塞性肺疾病急性发作中的免疫调节作用 被引量:1

Immunomodulatory role of interleukin-1 receptor-associated kinase M during acute exacerbation of chronic obstructive pulmonary disease

摘要 目的探讨白细胞介素-1受体相关激酶-M(interleukin-1 receptor associated kinases M,IRAK-M)在慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)对炎症反应的免疫调节作用。方法选择2019年5月至2021年5月在四川省人民医院老年呼吸科住院明确诊断为AECOPD患者30例纳入A组,门诊随访的慢性阻塞性肺疾病稳定期患者30例纳入B组,在本院进行健康体检的体检者30例纳入C组。抽血三组研究对象静脉血,提取单核细胞,应用实时荧光定量PCR仪检测各组研究对象单核细胞中IRAK-M、Toll样受体4(Toll-like receptor 4,TLR-4)的mRNA表达,采用酶联免疫吸附法检测各组研究对象血清中白细胞介素-6(interleukin-6,IL-6)、白细胞介素-10(interleukin-10,IL-10)、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)水平。结果AECOPD患者单核细胞中IRAK-M、TLR4的mRNA表达量均明显高于稳定期组及健康对照组,差异均有统计学意义(P<0.01);慢阻肺稳定期组及健康对照组IRAK-M、TLR4的mRNA表达量差异无统计学意义(P>0.05)。AECOPD患者时血清IL-6、IL-10和hs-CRP水平明显高于稳定期组及健康对照组,差异均有统计学意义(P<0.01),慢阻肺稳定期组与健康对照组血清IL-6、IL-10和hs-CRP水平比较,差异均无统计学意义(P>0.05)。结论在慢阻肺急性发作时,机体可能通过上调IRAK-M水平来调控炎症反应强度,从而避免过强的炎症反应对机体的伤害。 Objective To explore the immunomodulatory effect of interleukin-1 receptor-associated kinase M(IRAK-M)during acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Forty patients with AECOPD,40 stable patients with COPD,and 30 healthy controls were studied.The levels of IRAK-M and TLR4 mRNA in monocytes and the levels of interleukin-6(IL-6)、interleukin-10(IL-10)、and high-sensitivity C-reactive protein(hs-CRP)in serum were determined.Results The levels of IRAK-M and TLR4 mRNA expression in monocytes obtained from the AECOPD group were significantly higher than the levels in the stable and healthy control groups(P<0.01).Similarly,the serum levels of IL-6,IL-10,and hs-CRP were significantly higher in the patients with AECOPD than the stable patients with COPD and the healthy control group(P<0.01).There was no significant difference in above inflammatory indexes between the stable COPD and healthy control groups(P>0.05).Conclusion During AECOPD,the body may control the intensity of the inflammatory response by up regulating the level of IRAK-M to avoid the damage of an excessive inflammatory response.
