营养管理对肺结核患者营养状况及治疗效果的影响

Influence of nutrition management on nutritional status and therapeutic effect of patients with pulmonary tuberculosis

摘要 目的探讨营养管理对肺结核伴有营养不良患者的应用效果。方法选取2020年3月—2023年12月新疆维吾尔自治区塔城地区人民医院收治的312例菌阳肺结核患者,均应用营养风险筛查2002(nutrition risk screening,NRS2002)进行评估,筛查存在营养风险的患者,根据主观综合评定法(subjective global assessment,SGA)对患者进行评估,根据SGA评分将患者分为无营养不良组60例及伴有营养不良组60例,比较两组患者入院时及入院4周后的NRS2002评分变化及总蛋白、白蛋白、血红蛋白、红细胞比容、平均红细胞体积变化情况,并对两组患者的各项指标进行比较。结果实施营养管理4周后,患者的NRS2002评分、SGA评分及总蛋白、白蛋白、血红蛋白、红细胞比容、平均红细胞体积变化与入院时比较,差异有统计学意义(P<0.05)。肺结核伴有营养不良组的肢体水肿情况、总蛋白水平、饮食量、胃肠道症状、红细胞比容、白蛋白、结核菌检测及咳嗽咳痰症状的改善效果高于肺结核未伴有营养不良组(z=-9.470、-4.656、-3.012、-3.110、0.003、-2.099、-2.205、-4.073,均P<0.05)。结论营养管理有效改善了伴有营养不良及存在营养风险的肺结核患者的营养状况,临床效果确切。 Objective To explore the effect of nutrition management on pulmonary tuberculosis patients with malnutrition.Methods In March 2020 to December 2023,selected into areas of Xinjiang Uygur autonomous region Tacheng District People's Hospital treated 312 cases of bacteria positive tuberculosis patients.application of nutritional risk screening 2002(NRS2002)assessment,screening of nutritional risk patients,A comprehensive evaluation method is based on the subjective(SGA)to evaluate patients,according to the SGA score,divided the patients into no malnutrition group of 60 cases with malnutrition group and 60 cases.Compare the two groups of patients admitted to hospital and admitted to hospital after four weeks of NRS2002 score changes and total protein,albumin,hemoglobin,red blood cell volume,the average red blood cell volume change situation,and carries on the comparison to each index of two groups of patients.Results Four weeks after the implementation of nutritional management,the patient's NRS2002 score,SGA score and total protein,albumin,hemoglobin,red blood cell volume,average red blood cell volume change compared with on admission,the difference was statistically significant(P<0.05).The improvement effect of limb edema,total protein level,dietary intake,gastrointestinal symptoms,hematocrit,albumin,tuberculosis bacteria and cough and sputum symptoms in the pulmonary tuberculosis group was higher than that in the pulmonary tuberculosis without malnutrition group(z=-9.470,-4.656,-3.012,-3.110,0.003,-2.099,-2.205,-4.073,all P<0.05).Conclusion Nutritional management has effectively improved the nutritional status of pulmonary tuberculosis patients with malnutrition and nutritional risk,and the clinical application effect is accurate.
