摘要 背景:前交叉韧带(ACL)重建是常见的手术,目前存在多种固定方式。目的:探讨自体腘绳肌腱移植股骨与胫骨端采用RigidFix固定重建前交叉韧带的疗效。方法:选取2007年10月至2017年10月采用自体腘绳肌腱移植股骨与胫骨两端RigidFix双横钉固定重建ACL的患者129例(131膝),其中男105例(107膝),女24例(24膝)。年龄11~54岁,平均(29.2±9.7)岁。采用国际膝关节评分委员会(IKDC)、Lysholm、Tegner评分及Rolimeter测量膝关节前向松弛度(SSD)评价该固定方式的疗效。结果:所有患者随访24~32个月,平均(25.8±1.8)个月,末次随访IKDC评分83.8±7.1,Lysholm评分88.0±5.2,Tegner评分5.5±0.9,Rolimeter测量胫骨前移SSD(2.1±1.1)mm,与术前相比,差异军有统计学意义(P<0.001)。无关节感染、无韧带松弛、无翻修病例。结论:自体腘绳肌腱移植股骨与胫骨端采用RigidFix双横钉固定重建ACL避免了对移植肌腱的挤压切割,有利于腱骨接触和愈合,是一种安全有效的固定方式。 Background:Anterior cruciate ligament reconstruction is a common operation and there are many fixation methods.Objective:To investigate the effect of RigidFix on anterior cruciate ligament reconstruction with autogenous hamstring tendon grafts.Methods:From October 2007 to October 2017,a total of 129 patients(131 knees)received anterior cruciate ligament reconstruction by double RigidFix screws at both ends of the femur and tibia with autologous hamstring tendon grafts were selected in the study including 105 males(107 knees)and 24 females(24 knees)with the mean age of(29.2±9.7)years(range,11-54).The evaluation methods were knee scores like International Knee Documentation Committee(IKDC),Lysholm and Tegner and instrumented laxity measurements(side to side difference,SSD).Results:All patients were followed up for 24-32 months after the operation,with an average of(25.8±1.8)months.IKDC score was 83.8±7.1,Lysholm score was 88.0±5.2,Tegner score was 5.5±0.9 and the SSD measured by Rolimeter was(2.1±1.1)mm at the last follow-up,and compared with that before operation all these indexes had siginificant difenrence(P<0.001).There was no joint infection,no ligament relaxation and no revision patients.Conclusions:Anterior cruciate ligament reconstruction with autologous hamstring tendon fixed by RigidFix in femoral and tibial tunnel is feasible,avoiding the compression and cutting of the transplanted tendon,which is beneficial to the contact and healing of tendon and bone.
