摘要
目的探讨MRI功能成像对肌层浸润性膀胱癌(MIBC)的诊断价值。方法选取2017年7月至2019年6月间锦州医科大学附属第一医院收治的39例膀胱癌患者,根据肿瘤类型不同分为肌层浸润性膀胱癌(MIBC)组与非浸润性膀胱癌(NMIBC)组,MIBC组23例,NMIBC组16例。所有患者术前行MRI检查,包括T2加权成像和扩散加权成像序列,对MIBC及NMIBC进行诊断,并测量表观扩散系数(ADC)值,制作受试者工作曲线。结果T2WI+DWI序列诊断MIBC型膀胱癌22例,NMIBC型17例。MRI诊断是否肌层浸润曲线下面积为0.766,曲线下面积95%CI为0.608~0.925,ADC值诊断是否肌层浸润曲线下面积为0.845,曲线下面积95%CI为0.710~0.980,Cut-off值为0.950×10^-3mm^2/s,差异均有统计学意义(均P<0.05)。以手术病理结果为金标准,MRI ADC的准确率、敏感度和特异度分别为82.1%、82.6%和81.3%。结论MRI ADC值阈值对肌层浸润性膀胱癌诊断具有一定的临床意义和应用价值。
Objective To study the diagnostic value of functional magnetic resonance imaging(MRI)for myometrial invasive bladder cancer(MIBC).Methods Thirty-nine patients with bladder cancer who were treated at the First Affiliated Hospital of Jinzhou Medical University from July 2017 to June2019 were enrolled.They were divided into a MIBC group(23 patients)and a non-myometrial invasive bladder cancer(NMIBC group,16 patients).MRI was performed on all the patients including T2-weighted imaging(T2 WI)and diffusion weighted image(DWI)and MIBC and NMIBC were diagnosed.Apparent diffusion coefficient(ADC)was measured and receiver operating characteristic(ROC)curve was made.Results Twenty-two patients were diagnosed with MIBC and 17 patients were diagnosed with NMIBC by T2 WI plus DWI.The area under the curve(AUC)for muscular infiltration was 0.766(95%CI,0.608-0.925)by MRI.The AUC for muscular infiltration was 0.845(95%CI,0.710-0.980)by ADC.The Cutoff was 0.950×10^-3 mm^2/s(all P<0.05).When surgical pathology report was consider as gold standard,the accuracy,sensitivity and specificity was 82.1%,82.6%and 81.3%,respectively.Conclusion The ADC threshold of MRI has certain clinical significance and application value in the diagnosis of myometrial invasive bladder cancer.
