摘要
目的探讨微创拔牙后牙缺失中的种植体即刻种植修复效果。方法选取2018年7月至2019年7月我院收治的80例微创拔牙患者,微创拔牙后,即刻植入种植体,并于治疗后3~6个月进行永久固定义齿修复,随访患者牙龈乳头、牙槽骨吸收高度以及种植体固定情况,综合评判种植体修复效果。结果(1)早期成功率:所有患者于术后(14.9±5.3)个月进行随访,所有患者修复早期均达到了良好的稳定效果,且都实施了固定义齿修复,早期成功率为100%,患者对种植牙的外形及功能都满意。(2)随访情况:随访期间发现75例患者种植体牢靠无松动迹象,牙龈乳头指数为Ⅲ~Ⅳ,牙槽骨的吸收高度<2 mm,评估结果为种植成功;3例患者出现了龈缘充血问题,未出现溢脓,龈袋深度为3 mm,牙龈乳头指数为Ⅱ级,牙槽骨的吸收高度2.5 mm,种植牙出现轻度松动,评估结果为基本成功;有2例患者出现龈缘红肿溢肿问题,牙龈指数为Ⅰ级,牙槽骨吸收高度达到3 mm,种植体有较为明显的松动迹象,达到Ⅱ级,评估结果为失败,为患者实施临床拔除。最后,随访期间患者种植体的成功率为97.50%。结论微创拔牙后种植即刻种植修复牙缺失手术成功率、种植成功率理想,效果显著。
Objective To investigate the effect of immediate implant restoration on missing teeth after minimally invasive extraction.Methods Eighty patients with minimally invasive extraction were selected from July 2018 to July 2019 in our hospital.After minimally invasive extraction,implants were immediately implanted,and permanent fixed denture restoration was carried out 3~6 months after treatment.The gingival papilla,alveolar bone absorption height and implant fixation were followed up to comprehensively evaluate the implant repair effect.Results(1)Early success rate:all patients were followed up for(14.9±5.3)months after surgery,all patients achieved good stability in the early stage of restoration,and all of them implemented fixed denture restoration,the early success rate was 100%,patients were satisfied with the shape and function of implant teeth.(2)Follow up:during the follow-up period,75 patients were found to have firm implants without signs of loosening.The gingival papilla index wasⅢ~Ⅳ,and the alveolar bone absorption height was less than 2 mm.The evaluation results showed that the implantation was successful.Three patients had gingival edge congestion,no pus overflow,the gingival pocket depth was 3 mm,the gingival papilla index was gradeⅡ,the alveolar bone absorption height was 2.5 mm,and the implant teeth were slightly loose,the results were basically successful.There were 2 patients with gingival margin swelling and swelling,the gingival index was gradeⅠ,the alveolar bone absorption height reached 3 mm,the implant had obvious signs of loosening,reaching gradeⅡ,the evaluation result was failure,and the clinical extraction was implemented for the patients.Finally,the success rate of implant was 97.50%.Conclusion The success rate and implant success rate of immediate implant restoration after minimally invasive extraction are ideal,and the effect is remarkable.
