摘要
目的观察传统中医药学祛风活血通络法联合尼莫地平治疗颅内动脉瘤破裂蛛网膜下腔出血(aSAH)后迟发性脑血管痉挛(CVS)的临床疗效。方法选取Hunt-Hess分级为Ⅰ-Ⅳ级aSAH患者48例,随机分为对照组和治疗组,每组24例,对照组在常规治疗的基础上给予尼莫地平治疗,治疗组在对照组的基础上再加用传统医学祛风活血通络法治疗,对比两组患者在发病后第1天、第7天、第14天迟发性脑血管痉挛和脑梗死的发生率以及大脑中动脉平均血流速度(MBF)、同侧大脑中动脉与颈内动脉颅外段血流速度的比值(Li指数)。结果治疗组迟发性脑血管痉挛和脑梗死的发生率、大脑中动脉平均血流速度、Li指数在第7、14天明显低于对照组(P<0.05)。结论传统医学祛风活血通络法联合尼莫地平能够有效降低aSAH后迟发性CVS和脑梗死发生率,相较于单纯尼莫地平治疗,疗效更优,有助于改善患者的预后。
Objective To observe the clinical effect of Qufeng Huoxue Tongluo combined Nimodipine for treating delayed cerebral vasospasm(CVS)after aneurvsmal subarachnoid hemorrhage(aSAH).Methods Forty-eight aSAH patients with Hunt-Hess gradeⅠ-Ⅳwere randomly assigned to a control group and a treatment group,24 in each group.Patients in the control group took Nimodipine on the basis of conventional treatment.Those in the treatment group additionally took traditional Chinese medicine decoction of Qufeng Huoxue Tongluo.The probability event of delayed CVS and cerebral infarction on the 1st,7th and 14th day after attack,as well as mean cerebral blood flow velocity(MBF),ipsilateral middle cerebral artery or internal carotid artery extracranial blood flow velocity(Lindegaard,Li)index.Results The probability event of delayed CVS and cerebral infarction,MBF,Li index in the treatment group were significantly lower than those in the control group on the 7th and 14th day(P<0.05).Conclusion Qufeng Huoxue Tongluo combined Nimodipine effectively reduced the probability event of delayed CVS and cerebral infarction after aSAH.Compared with nimodipine alone,Qufeng Huoxue Tongluo combined with nimodipine is more effective and helpful to improve the prognosis of patients.
