摘要
目的从卫生经济学评价社区结肠癌筛查方案的实施价值。方法于2016年5~8月任意选取江门市辖区内10000名年龄在40~55岁、无肠道症状、无心脑血管等基础疾病的医保参保居民进行调查研究,采用随机抽签法将其随机分为筛查组和观察组,每组各5000名。两组进行为期5年的肠道疾病随访调查,在此期间,观察组不进行社区肠癌筛查,患者有不适肠道症状就诊进行治疗;筛查组在研究期第一年内进行结肠镜检查和针对性治疗。比较两组5年内结肠息肉、结肠癌发生率以及5年内总医保报销费用的差异。结果两组结肠息肉对比,差异无统计学意义(P>0.05),但筛查组结肠癌发生率显著低于观察组(P<0.05)。5年内筛查组总医保报销费用显著少于对照组,且成本效益比为1.29:1。结论社区结肠癌筛查策略具有成本低、效果佳、效益高等优点,能够有效防治结肠癌,减轻患者的经济负担。
Objective To evaluate the implementation value of community colon cancer screening program from the perspective of health economics.Methods From May 2016 to August 2016,10000 cases of insured residents aged 40~55,without intestinal symptoms,cardiovascular and cerebrovascular diseases in Jiangmen City were randomly selected for investigation.They were randomly divided into screening group and observation group by random drawing,with 5000 people in each group.The two groups were followed up for 5 years.During this period,the observation group was not screened for colorectal cancer in community,patients with intestinal symptoms were treated.The screening group received colonoscopy and targeted treatment in the first year of the study period.The incidence of intestinal polyps and colorectal cancer within 5 years and the total medical insurance reimbursement expenses within 5 years were compared between the two groups.Results There was no significant difference between the two groups(P>0.05),but the incidence of colon cancer in the screening group was significantly lower than that in the observation group(P<0.05).Within 5 years,the total medical insurance reimbursement cost of the screening group was significantly less than that of the control group,and the cost-benefit ratio was 1.29:1.Conclusion Community colon cancer screening strategy has the advantages of low cost,good effect and high benefit,which can effectively prevent and treat colon cancer and reduce the economic burden of patients.
