摘要
目的探讨分析碎骨钻孔肌腱线捆绑联合克氏针内固定治疗手指甲粗隆粉碎性开放性骨折的近远期效果。方法选择2019年6月-2020年6月我院收治的64例手指甲粗隆粉碎性开放性骨折患者为研究对象,根据手术方案不同分为2组,每组各32例。其中,对照组采用传统方法处理骨髓块联合复位固定治疗,观察组行碎骨钻孔肌腱线捆绑联合克氏针内固定术,比较两组术后3个月的疗效、骨折愈合时间、术后并发症发生情况,并于术后12个月对患者进行随访了解其手功能恢复情况。结果术后3个月,观察组的治疗总有效率显著高于对照组,骨折愈合时间明显短于对照组,且术后并发症发生率明显低于对照组(P<0.05)。术后12个月随访,观察组的Jebsen手功能测试结果明显优于对照组(P<0.05)。结论碎骨钻孔肌腱线捆绑联合克氏针内固定治疗手指甲粗隆粉碎性开放性骨折可获得良好的近远期效果,具有骨折愈合时间短、并发症发生率低、手部功能恢复佳等优势,值得推广应用。
Objective To investigate and analyze the short-term and long-term effect of bone broken drilling tendon binding combined with Kirschner wire internal fixation in the treatment of Comminuted Open nail trochanter fracture.Methods 64 cases of Manicure trochanter Comminuted Open Fracture in our hospital were selected as the research object.The patients were divided into two groups according to the operation plan,32 cases in each group.The control group was treated with traditional treatment of bone marrow block combined with reduction and fixation,while the observation group was treated with broken bone drilling tendon binding combined with Kirschner wire internal fixation.The curative effect,fracture healing time,postoperative complications and hand function recovery of the two groups were compared.Results Three months after operation,the total effective rate of the observation group was significantly higher than the control group,the fracture healing time and the incidence of postoperative complications were significantly lower than the control group(P<0.05).At 12 months after operation,Jebsen hand function test scores of the observation group were significantly better than the control group(P<0.05).Conclusion It has the advantages of short fracture healing time,low complication rate and good hand function recovery,which is worthy of popularization and application.
