摘要
目的探讨乳腺肿块良恶性鉴别诊断中经皮粗针穿刺活检的应用价值。方法以我院2017年11月-2020年10月于乳腺门诊就诊的63例乳腺肿块患者为研究对象,均进行经皮粗针穿刺活检病理检查,并将该结果与肿块切除标本的常规病理检查即开放活检结果进行比较,分析不同检查方法的应用价值。结果将63例乳腺肿块患者的经皮粗针穿刺活检病理检查结果与开放活检结果进行对比,Kappa值为0.81,二者在良恶性鉴别诊断方面具有较强的一致性;其中经皮粗针穿刺活检与开放活检病理诊断符合率为98.41%,漏诊率为1.59%;患者经皮粗针穿刺活检术后均未出现感染、局部血肿、活动性出血等穿刺所致并发症。结论乳腺肿块采取超声引导下经皮粗针穿刺活检与开放性活检病理诊断的一致性较高,在良恶性鉴别诊断中具有重要临床价值。
Objective To explore the value of percutaneous needle biopsy in the differential diagnosis of benign and malignant breast masses.Methods A total of 63 patients with breast masses in our hospital from November 2017 to October 2020 in the breast clinic were selected as the research objects.All of them underwent percutaneous needle biopsy pathological examination,and the results were compared with the routine pathology of the mass excised specimens.Inspection is to compare the results of open biopsy and analyze the application value of different inspection methods.Results Comparing the pathological examination results of the percutaneous needle biopsy with the open biopsy results of 63 patients with breast masses,the Kappa value was 0.81,and the two had a strong consistency in the differential diagnosis of benign and malignant;among them,the percutaneous thick needle The coincidence rate of the pathological diagnosis between needle biopsy and open biopsy was 98.41%,and the missed diagnosis rate was 1.59%.There were no complications caused by infection,local hematoma,active bleeding,etc.after percutaneous needle biopsy.Conclusion Ultrasound-guided percutaneous needle biopsy is consistent with open biopsy in pathological diagnosis of breast masses,which has important clinical value in the differential diagnosis of benign and malignant tumors.
