摘要
目的浅析小容量肺灌洗对煤工尘肺合并稳定期慢性阻塞性肺疾病患者的临床疗效。方法纳入本单位于2018年1月-2019年12月收治的48例煤工尘肺合并稳定期慢性阻塞性肺疾病患者进行研究,根据随机数字表法分为观察组(小容量肺灌洗治疗)、对照组(常规药物治疗),每组各24例,评价两组患者治疗效果。结果治疗前两组第1秒钟用力呼吸量(FEV1)、用力肺活量(FVC)、每分通气量(VE)值、MMP-9、TGF-β1、TNF-α水平值、SGRQ、6MWT结果比较均无统计学意义(P>0.05),治疗后两组FEV1、FVC、VE值比较无统计学意义(P>0.05),治疗后观察组MMP-9、TGF-β1、TNF-α水平值、SGRQ评分均低于对照组、观察组6MWT值高于对照组(P<0.05)。结论煤工尘肺合并稳定期慢性阻塞性肺疾病患者给予小容量肺灌洗治疗可减轻机体炎症反应,提高生活质量与活动耐量。
Objective To analyze clinical effect of small volume lung lavage on pneumoconiosis complicated with stable chronic obstructive pulmonary disease of coal workers.Methods The paper chose 48 coal worker with pneumoconiosis and stable chronic obstructive pulmonary disease treated in our unit from January 2018 to December 2019 for study,and divided them into observation group(with small volume lung lavage treatment)and control group(with routine drug treatment)according to the random number table method,with 24 cases in each group.Treatment effect between two groups was evaluated.Results Before treatment,forced respiration volume(FEV1),forced vital capacity(FVC),minute ventilation(VE),MMP-9 and TGF-β1,TNF-αlevel,SGRQ and 6MWT during the first second showed no significant difference between two groups,(P>0.05).There was no significant difference in FEV1,FVC and VE between two groups after treatment,(P>0.05).MMP-9 and TGF-β1,TNF-αlevel and SGRQ scores in observation group after treatment were lower than control group,6MWT value of observation group was higher than control group(P<0.05).Conclusion Small volume lung lavage can reduce inflammatory response and improve life quality and activity tolerance of coal worker with pneumoconiosis and stable chronic obstructive pulmonary disease.
