摘要
目的分析社区卫生服务中心高血压糖尿病慢性病管理模式对患者血压或血糖水平控制的作用。方法选择本社区卫生服务中心2018年8月-2021年1月接收的60例高血压糖尿病患者作为研究对象,采用数字表法将其分为参照组与研究组,每组各30例,参照组采用常规慢性疾病管理,研究组采用优质慢性病管理,观察和比较两组管理前后血糖、血压控制效果。结果管理前,两组血压水平、血糖水平比较,差异无统计学意义(P>0.05);管理后,研究组血压水平、血糖水平低于参照组,差异具有统计学意义(P<0.05)。结论采用社区卫生服务中心高血压糖尿病慢性病管理模式,可有效增强患者血压、血糖水平控制效果。
Objective To analyze the effect of hypertension diabetes chronic disease management mode on blood pressure and blood sugar level in community health service centers.Methods 60 patients with hypertension and diabetes received from August 2018 to January 2021 in the community health service center were selected as the research objects.They were divided into the reference group and the research group by digital table method.Each group had 30 cases.The reference group was treated with conventional chronic disease management,while the study group adopted the management of chronic disease with good quality.The control effects of blood sugar and blood pressure before and after the two groups were observed and compared.Results Before management,there was no significant difference in blood pressure and blood glucose between the two groups(P>0.05);After management,the blood pressure and blood glucose levels in the study group were lower than those in the reference group,and the difference was statistically significant(P<0.05).Conclusion The management mode of hypertension and diabetes chronic disease in community health service center can effectively enhance the control effect of blood pressure and blood sugar level.
