摘要
目的探究龈下菌斑人类巨细胞病毒(HCMV)、EB病毒(EBV)感染对慢性牙周炎患者龈沟液白细胞介素(IL-1、IL-6、IL-8)及其mRNA水平和基质金属蛋白酶/组织金属蛋白酶抑制剂(MMPs/TIMPs)平衡的影响。方法选取2016年1月-2019年4月入院诊治的慢性牙周炎患者206例,根据患者病情严重程度分为轻度、中度、重度,并募集同期做口腔检查的健康志愿者(55名)为对照组,比较各组HCMV、EB感染率。根据龈下菌斑是否出现HCMV、EBV感染,进一步分为HCMV(+)、HCMV(-)与EBV(+)、EBV(-)患者,比较各亚组龈沟液中IL-1、IL-6、IL-8、MMP-1、MMP-8、MMP-9及TIMP-1的表达。结果慢性牙周炎组HCMV、EBV感染检出率分别为37.86%(78/206)、57.77%(119/206),分别高于对照组(P<0.05);随着病情严重程度的增加,HCMV、EBV感染检出率上升(P<0.05)。HCMV(+)患者龈沟液中IL-1β、IL-6、IL-8蛋白及mRNA、MMP-1、MMP-8、MMP-9表达均高于HCMV(-)患者,TIMP-1表达低于HCMV(-)患者(P<0.05)。EBV(+)患者龈沟液中IL-1β、IL-6、IL-8蛋白及mRNA、MMP-1、MMP-8、MMP-9表达均高于EBV(-)患者,TIMP-1表达低于EBV(-)患者(P<0.05)。结论HCMV、EBV感染会加重慢性牙周炎患者病情,可能与加剧口腔局部炎症反应和诱导MMPs/TIMPs表达紊乱有关。
OBJECTIVE To explore the effects of human cytomegalovirus(HCMV)and EB virus(EBV)infection of subgingival plaque on protein and mRNA expression levels of interleukin(IL-1,IL-6,IL-8)in gingival crevicular fluid(GCF),and balance between matrix metalloproteinases and tissue inhibitor of matrix metalloproteinases(MMPs/TIMPs)in patients with chronic periodontitis.METHODS A total of 206 patients with chronic periodontitis treated in the hospital from Jan 2016 to Apr 2019 were enrolled and divided into the mild,moderate and severe groups according to disease severity.Another 55 healthy volunteers undergoing oral examination during the same period were enrolled as the control group.The infection rates of HCMV and EBV in groups were compared.According to presence or absence of HCMV and EBV infection in subgingival plaque,they were furtherly divided into the HCMV(+)group,HCMV(-)group,EBV(+)group and EBV(-)group.The expression levels of interleukins(IL-1,IL-6,IL-8),MMP-1,MMP-8,MMP-9 and TIMP-1 in GCF were compared in all subgroups.RESULTS The detection rates of HCMV and EBV infection in the chronic periodontitis group were 37.86%(78/206)and 57.77%(119/206),respectively,which were significantly higher than those in the control group(P<0.05).With the exacerbation of disease,detection rates of HCMV and EBV infection increased significantly(P<0.05).The protein and mRNA expression levels of IL-1β,IL-6 and IL-8,and protein levels of MMP-1,MMP-8 and MMP-9 in GCF in the HCMV(+)group were significantly higher than those in the HCMV(-)group,while expression of TIMP-1 was significantly lower than that in the HCMV(-)group(P<0.05).The protein and mRNA levels of IL-1β,IL-6 and IL-8,and protein expression of MMP-1,MMP-8 and MMP-9 in GCF in the EBV(+)group were significantly higher than those in the HCMV(-)group,while expression of TIMP-1 was significantly lower than that in the EBV(-)group(P<0.05).CONCLUSION HCMV and EBV infection will aggravate the conditions of patients with chronic periodontitis,which may be related to oral local inflammatory response and disordered balance of MMPs/TIMPs expression.
