双源CT前瞻性心电门控大螺距Flash扫描模式在支气管动脉CTA成像中的应用 被引量:2

摘要 目的探讨双源CT前瞻性心电门控大螺距扫描模式在支气管动脉CTA成像中的应用价值。方法选择2017年7月-2020年1月在浙江省丽水市中心医院用西门子第三代双源Force CT行支气管动脉CTA检查的患者90例,随机分为三组,每组30例,分别采用非心电门控螺旋扫描模式、非心电门控大螺距扫描模式及前瞻性心电门控大螺距扫描模式扫描,比较三组辐射剂量、主观及客观图像成像质量,客观评价图像质量包括胸主动脉CT值、噪声(SD)、信噪比(SNR)、对比噪声比(CNR)。结果辐射剂量方面,心电大螺距组DLP、CTDI_(vol)、ED低于非心电螺旋组及非心电大螺距组,差异均有统计学意义(P<0.05);非心电大螺距组DLP、CTDI_(vol)、ED低于非心电螺旋组,差异均有统计学意义(P<0.05)。心电大螺距组图像质量主观优良率高于非心电螺旋组及非心电大螺距组,差异均有统计学意义(P<0.05);非心电大螺距组图像质量主观优良率高于非心电螺旋组,差异有统计学意义(P<0.05)。三组图像主动脉CT值均能满足临床诊断要求,且三组主动脉图像CT值、SD值、SNR、CNR比较差异均无统计学意义(P>0.05)。结论在心率<90次/min的患者中,应用双源CT前瞻性心电门控大螺距Flash扫描模式进行支气管动脉CTA成像,图像质量与非心电门控下螺旋扫描和大螺距Flash扫描相比明显提高,且辐射剂量大幅度减少,仅为(0.66±0.07)mSv,具有较大的应用和推广价值。 Objective To evaluate the value of dual source CT large pitch prospective ECG gated scanning mode in bronchial artery CTA imaging.Methods 90 patients who underwent bronchial artery CTA with Siemens third-generation dual source force CT in Lishui Central Hospital of Zhejiang Province from July 2017 to January 2020 were randomly divided into three groups,with 30 cases in each group.They were scanned with non ECG gated spiral scanning mode,large pitch non ECG gated scanning mode and large pitch prospective ECG gated scanning mode respectively.The radiation doses of the three groups were compared Subjective and objective image quality.Objective evaluation of image quality includes thoracic aorta CT value,noise(SD),signal-to-noise ratio(SNR)and contrast noise ratio(CNR).Results In terms of radiation dose,DLP,CTDIvol and ed in ECG group were lower than those in non ECG spiral group and non ECG large pitch group(P<0.05);DLP,CTDIvol and ed in non ECG large pitch group were lower than those in non ECG spiral group(P<0.05).The subjective excellent and good rate of ECG large pitch group was higher than that of non ECG spiral group and non ECG large pitch group(P<0.05);The subjective excellent and good rate of image quality in non ECG large pitch group was higher than that in non ECG spiral group(P<0.05).The CT value of aorta in the three groups could meet the requirements of clinical diagnosis,and there was no significant difference in CT value(HU),SD value,SNR and CNR value among the three groups(P>0.05).Conclusion In patients with heart rate<90 beats/min,using dual source CT prospective ECG gated large pitch flash scanning mode for bronchial artery CTA imaging,the image quality is significantly improved compared with non ECG gated spiral scanning and large pitch flash scanning,and the radiation dose is significantly reduced,only(0.66±0.07)mSv,which has great application and popularization value.
