摘要
目的探讨房颤患者左心耳血栓评价中经食道超声声学造影的临床价值。方法回顾性选取2019年12月—2021年3月唐山市中心医院房颤患者74例,均接受经食道超声声学造影及增强CT检查,将临床诊断作为金标准,比较左心耳血栓和非左心耳血栓患者的左心耳各项参数、经食道超声声学造影及增强CT检查结果。结果左心耳血栓患者的LAA-min大于非左心耳血栓患者(P<0.05),LAA-max大于非左心耳血栓患者(P<0.05)。诊断结果中,阳性11例、阴性63例;经食道超声声学造影分别检出16、58例;增强CT分别检出31、43例。经食道超声声学造影的诊断价值高于增强CT(P<0.05)。结论经食道超声声学造影对房颤患者左心耳血栓的评价准确性较高,可推行。
Objective To investigate the clinical value of transesophageal contrast-enhanced ultrasound in the evaluation of left atrial appendage thrombosis in patients with atrial fibrillation.Methods A total of seventy-four cases of patients with atrial fibrillation in Tangshan Central Hospital from January 2019 to January 2020 were retrospectively selected,all of them received transesophageal contrast-enhanced ultrasound and enhanced CT examination,and the clinical diagnosis was taken as the gold standard.The parameters of left atrial appendage,transesophageal contrast-enhanced ultrasound and enhanced CT examination results of patients with left atrial appendage thrombosis and non left atrial appendage thrombosis were compared.Results The LAA-min and LAA-max of patients with left atrial appendage thrombosis were higher than patients without left atrial appendage thrombosis(P<0.05).Of the 74 cases of patients,11 cases were positive and 63 cases were negative in clinical diagnosis;16 cases were positive and 58 cases were negative in transesophageal contrast-enhanced ultrasound;31 cases were positive and 43 cases were negative in enhanced CT examination.The sensitivity,specificity,accuracy and positive predictive value of CEUS were higher than contrast-enhanced CT(P<0.05).Conclusion The clinical value of transesophageal contrast-enhanced ultrasound is higher than contrast-enhanced CT in the evaluation of left atrial appendage thrombosis in patients with atrial fibrillation.
