摘要
目的:评价药师对慢性病犯实施药学监管,对慢性病防治效果的影响。方法:将2022年3月—2023年2月内蒙古某监狱394名患高血压、糖尿病及脑梗、陈旧性心梗等心脑血管病的罪犯作为研究对象,随机分为两组并制定差异化干预方案,以慢病病情发展状况、门诊接诊率、急诊接诊率、住院率、转诊率及病死率为评测指标,跟踪检测病情。结果:药师干预对慢性病犯的管理与开展药学服务效果良好,有效降低门诊率、急诊率、住院率和转诊率,而对慢性病控制率显著提高。结论:通过药师与慢性病犯有效沟通,提高服药的依从性,增强用药信心,并对病犯出现的问题及时提出合理化建议,增加信任度,提高治疗信心和稳定改造情绪。在药师干预之下慢性病犯防治效果明显优于未进行干预。
Objective:To demonstrate the effect of pharmaceutical supervision on chronic disease prevention and treatment by pharmacists in the supervision place.Method:A total of 394 prisoners suffering from hypertension,diabetes,cerebral infarction,old myocardial infarction and other cardiovascular and cerebrovascular diseases in a prison in Inner Mongolia from March 2022 to February 2023 were selected as experimental subjects.They were randomly divided into two groups and differentiated intervention programs were formulated.The development of chronic disease,outpatient treatment rate,emergency treatment rate,hospitalization rate,referral rate and mortality were used as evaluation indexes.The change of the condition of the experimental samples was tracked and detected.Results:Pharmacists'intervention had a good effect on the management and pharmaceutical care of chronic patients,effectively reduced the outpatient rate,emergency rate,hospitalization rate and referral rate of chronic patients,and significantly improved the control rate of chronic diseases.Conclusion:Through effective communication between pharmacists and chronic patients,the medication compliance can be effectively improved,the patients'confidence in medication can be enhanced,and reasonable suggestions can be put forward timely to the problems of the patients,so as to increase the degree of trust of the patients,and promote their confidence in treatment and stable and reformed emotions.The effect of chronic disease prevention and treatment under the intervention of pharmacists is obviously better than that without intervention.
