摘要
目的探讨血清解偶联蛋白2(UCP2)水平对脓毒症的诊断价值和预后意义。方法选取2021年1月~2022年5月在四川省人民医院诊治的脓毒症患者104例(脓毒症组),根据28 d内预后情况将脓毒症患者分为存活组(n=68)与死亡组(n=36);另选取同期收治的局部感染患者100例作为对照组;检测患者入院24 h内血清UCP2、降钙素原(PCT)、C反应蛋白(CRP)、白细胞计数(WBC)、中性粒细胞(NEU)和血乳酸(Lac)等实验室指标;采用Spearman相关性检验对脓毒症患者血清UCP2水平与PCT水平进行相关性分析;采用多因素Logsitic回归模型分析各指标与脓毒症发生以及患者28 d预后的关系;并采用受试者工作特征(ROC)曲线评价各指标对脓毒症的诊断价值及预后预测价值。结果脓毒症组血清UCP2、PCT、CRP和Lac水平均显著高于对照组(P<0.05),两组WBC、NEU对比差异无统计学意义(P>0.05)。脓毒症患者血清UCP2水平与PCT水平呈正相关(r=0.351,P<0.05)。Logisitic回归分析显示,UCP2、PCT是脓毒症发生的独立危险因素(P<0.05),且是患者28 d死亡的独立危险因素(P<0.05)。ROC曲线分析显示,UCP2诊断脓毒症发生和预测患者28 d死亡的曲线下面积(AUC)分别为0.937、0.711。结论血清UCP2是脓毒症诊断的潜在标志物,其检测对于患者预后有预测意义。
Objective To investigate the diagnostic value and prognostic significance of serum uncoupling protein 2(UCP2)level in sepsis.Methods 104 patients with sepsis(sepsis group)who were treated in our hospital from January 2021 to May 2022 were selected.According to the prognosis within 28 days,the patients with sepsis were divided into survival group(n=68)and death group(n=36).In addition,100 patients with local infection admitted to our hospital during the same period were selected as the control group.Serum UCP2,procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC),neutrophil(NEU)and blood lactic acid(Lac)were detected within 24 hours after admission.Spearman correlation test was used to analyze the correlation between serum UCP2 level and PCT level in patients with sepsis.Multivariate Logsitic regression model was used to analyze the relationship between each index and the occurrence of sepsis and 28 d prognosis of patients.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value and prognostic value of each index for sepsis.Results The levels of serum UCP2,PCT,CRP and Lac in sepsis group were significantly higher than those in control group(P<0.05).Serum UCP2 level was positively correlated with PCT level in patients with sepsis(r=0.351,P<0.05).Logisitic regression analysis showed that UCP2 and PCT were independent risk factors for sepsis(P<0.05),and independent risk factors for 28-day death(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of UCP2 in the diagnosis of sepsis and the prediction of 28-day mortality were 0.937 and 0.711.Conclusion Serum UCP2 is a potential marker for the diagnosis of sepsis,and its detection has predictive significance for the prognosis of patients.
