摘要
目的探讨体外循环与非体外循环下进行心脏搭桥术的疗效。方法选取在我院行心脏搭桥术治疗的患者168例,根据不同手术方案分为两组,每组84例,对照组实施体外循环,试验组实施非体外循环。观察两组手术前后肌酸激酶同工酶(CK)、心肌肌钙蛋白I(cTnI)水平,记录两组手术相关指数。结果手术4h后,两组CKMB、cTnI水平均高于手术前,试验组低于对照组(P<0.05)。试验组手术持续时间、术后恢复自主呼吸时间和出院时间均短于对照组,术中失血量少于对照组(P<0.05)。结论和体外循环相比,非体外循环下进行心脏搭桥术对患者心肌损伤较小,有助于提高预后效果,值得进一步推广。
Objective To explore the effect of cardiopulmonary bypass and off-pump cardiac bypass.Methods A total of 168 patients who was treated by heart bypass surgery in our hospital were selected and were categorized to two groups based on the reasons for the different surgical plans,the control group with cardiopulmonary bypass and the test group with non-pulmonary bypass,each group 84 case.The creatine kinase isoenzyme(CK)and cardiac troponin I(cTnI)were observed and the operation-related indexes in the two groups were recorded before and after operation.Results The levels of CK-MB and cTnI in both groups were all higher than those before surgery and the test group was lower than the control group in 4 hours after surgery(P<0.05).The operation duration,postoperative recovery of spontaneous breathing time and discharge time in the test group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group(P<0.05).Conclusion Cardiac bypass surgery without extracorporeal circulation has less damage to the patient’s myocardium compared with cardiopulmonary bypass,which can help to improve the prognosis effect,worthy of further promotion.
