摘要
目的:探讨补肾通络汤联合钻孔减压、加压冲洗治疗Ⅰ、Ⅱ期股骨头坏死临床疗效及机制。方法:选取株洲市中医伤科医院就诊的Ⅰ、Ⅱ期股骨头坏死患者60例,随机分为治疗组和对照组。对照组予以钻孔减压联合康复训练;治疗组予以口服补肾通络汤、钻孔减压联合康复训练;对比两组治疗3个疗程前后的变化。结果:治疗组总有效率高于对照组总有效率,两组差异具有显著统计学意义(P<0.05)。治疗组Harris功能评分较对照组明显提高(P<0.05)。治疗组转化生长因子(Transforming Growth Factor,TGF)-β1及血清骨钙素(Bone Gla Protein,BGP)均较对照组显著升高(P<0.05)。结论:补肾通络汤联合钻孔减压、加压冲洗治疗Ⅰ、Ⅱ期股骨头坏死效果显著,可改善Harris功能评分,促进患者恢复,其作用机制可能与促进TGF-β1、BGP表达有关。
Objective:To investigate the clinical efficacy and mechanism of the Bushen Tongluo decoction(补肾通络汤)with borehole decompression and pressurized irrigation in the treatment of stageⅠandⅡosteonecrosis of the femoral head.Methods:A total of 60 patients with stageⅠandⅡfemoral head necrosis who attended Zhuzhou Hospital of Traditional Chinese Medicine and Traumatology Hospital were randomly divided in to the treatment group and the control group.The control group was treated by drilling decompression plus rehabilitation training.The treatment group was treated with the Bushen Tongluo decoction,drilling and decompression plus rehabilitation training.The changes before and after three courses of treatment in the two groups were compared.Results:The total effective rate in the treatment group was higher than that in the control group,and there was a significant difference between the two groups(P<0.05).The Harris function score in the treatment group was significantly higher than that in the control group(P<0.05).The treatment group transformed TGF-β1 and Serum osteocalcin BGP was significantly higher than that in the control group(P<0.05).Conclusion:The effect of the Bushen Tongluo decoction with drilling decompression and compression irrigation in the treatment of stageⅠandⅡfemoral head necrosis is significant,which can improve Harris function score and promote patient recovery,and its mechanism of action may be related to the promotion of TGF-β1 and BGP expression.
