摘要
患者男,63岁,后腰部暗红色斑块2年,伴轻微痒痛。皮损组织病理示:表皮角化过度伴角化不全,棘层增生肥厚,表皮各层可见核大胞浆透亮的细胞,并可见核大深染的异型细胞及角化不良细胞,真皮浅层较多炎性细胞浸润。免疫组化:CK7(-)、CK20(-)、CEA(-)、HER2(-)、GCDFP-15(-)、S100(-)、HMB45(-)、EMA(上皮+)、Ki-67(约40%+)。诊断:Paget样鲍温病。
A 63-year-old male presented with dark-red plaque on his back waist for two years, with mild itching and pain. The histopathology showed epidermal hyperkeratosis and parakeratosis, acanthosis, bright cytoplasmic cells with large nuclei and hyperchromatic atypia and dyskeratosis cell in each layer of epidermis, inflammatory cell infiltration in the superficial dermis. Immunohistochemistry showed CK7(-), CK20(-), CEA(-), HER2(-), GCDFP-15(-), S100(-), HMB45(-), EMA epithelial positive, Ki-67about 40% positive.Diagnosis: Pagetoid Bowen′s disease.
