摘要
目的:探究丙泊酚联合舒芬太尼在血液肿瘤患儿化疗鞘注中的应用价值。方法:选取我院2016年1月—2018年1月收治的需化疗鞘注的血液肿瘤患儿70例,按随机数字表法分为观察组和对照组,各35例。观察组采用丙泊酚联合舒芬太尼静脉麻醉,对照组采用利多卡因局麻。比较两组操作时间,一次穿刺成功率,镇静镇痛效果,患儿的苏醒时间、术中不良反应发生情况及患儿对再次操作的接受程度。结果:观察组操作时间短于对照组,差异有统计学意义(P<0.05);观察组一次穿刺成功率高于对照组,且术中哭闹、扭动、需要医务人员摁压等情况的发生少于对照组,差异有统计学意义(P<0.05);观察组患儿对再次操作的接受率为94.29%,高于对照组的42.86%,差异有统计学意义(P<0.05)。结论:采用丙泊酚联合舒芬太尼静脉复合麻醉进行无痛鞘注能够起到较好的镇静镇痛作用,有利于缩短操作时间,提高患儿配合度和一次操作成功率,且安全性较高,值得临床推广应用。
Objective:To explore the application value of propofol combined with sufentanil in chemotherapeutic intrathecal injection in children with hematological tumors.Method:70 children with hematological tumors requiring chemotherapeutic intrathecal injection admitted to our hospital from January 2016 to January 2018 were selected and randomly divided into an observation group and a control group,35 cases each.The children in the observation group were treated with propofol combined with sufentanil for intravenous anesthesia,and the children in the control group were treated with lidocaine for local anesthesia.The operation time,the success rate of one-time puncture,the effect of sedation and analgesia,the wake-up time of the children,the occurrence of adverse reactions during operation and the acceptance of reoperation were compared between the two groups.Result:The operation time in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.05).The success rate of one-time puncture in the observation group was higher than that in the control group,and the incidences of crying,writhing and pressing by medical staff during operation were less than those in the control group,with statistically significant differences(P<0.05).The acceptance rate of the reoperation in the observation group was 94.29%,which was higher than 42.86%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Intravenous anesthesia with propofol combined with sufentanil by chemotherapeutic intrathecal injection in children with hematological tumors can have a better sedative and analgesic effect,which can help shorten the operation time and improve the coordination degree of the children and the success rate of one-time operation,and has high safety,and is worthy of clinical popularization and application.
