摘要
目的评估不同月份痰菌结果对含利奈唑胺方案治疗的耐多药结核病患者治疗结局预测的有效性。方法选取广州市胸科医院肺结核科2014年10月1日—2017年11月1日使用含利奈唑胺方案治疗耐多药肺结患者,收集不同月份的痰涂片、痰培养结果,通过分析痰菌阴转情况,评价治疗结局预测的灵敏度、特异度和受试者工作特征曲线,比较不同月份痰菌结果对于治疗结局的预测效率。结果纳入47例患者中成功治疗40例;失败7例,其中2例死亡;成功率83.0%,十二月末痰培养转预测灵敏度(91.5%)、特异度(88.4%)、阳性预测值(95.0%)、ORC曲线下面积(0.832,P=0.005),优于三月末、六月末、九月末的相关指标,是最优的治疗结局预测指标。结论比较其他月份痰菌结果,十二月末的痰培养阴转能更有效的预测治疗结局。
Objective To evaluate the effectiveness of the results of sputum bacteria in different months in predicting the treatment outcome of MDR-TB patients treated with linezolid regimens.Methods The pulmonary tuberculosis department of Guangzhou Chest Hospital from October 1,2014 to November 1,2017 used linezolid-containing regimens to treat patients with multidrug-resistant lung nodes.Sputum smears and sputum culture results from different months were collected,the negative conversion of sputum bacteria was analyzed to evaluate the sensitivity,specificity and receiver operating characteristic curve of treatment outcome prediction,and compare the prediction efficiency of sputum bacteria results in different months for treatment outcome.Results Among 47 patients,40 were successfully treated;7 failed,and 2 of them died;the success rate was 83.0%.The sensitivity(91.5%),specificity(88.4%),positive predictive value(95.0%),the area under the ORC curve(0.832,P=0.005),which is better than the relevant indicators at the end of March,the end of June,and the end of September,and is the best predictor of treatment outcome.Conclusion Compared with the results of sputum bacteria in other months,the negative conversion of sputum culture at the end of December can more effectively predict the treatment outcome.
