Nowadays with the development of electrophysiological technique,more research began to focus on the role of gap junction in the neural network,especially in the cognitive function. Gap junction collaborates with chemical synapses to induce the neural network activities related to memory and perception,such as gamma and ripple oscillation,which is regarded as the mechanism of the role of gap junction in cognitive function. This review introduces the structure of gap junction,distribution and expression of different kinds of connexins in central nervous system,which are the basic element of gap junction. Furthermore,this review discusses the role of gap junction in cognition related network oscillations and activity-dependent plasticity of gap junction.展开更多

文摘目的借助循证医学方法,为1例初诊阿尔茨海默病患者确定治疗目标及治疗方案。方法充分评估患者情况,按照PICO原则,提出临床问题,文献检索资源:Cochrane图书馆(2010年第4期)、Cochrane Epilepsy GroupSpecialised Register、the Cochrane Epilepsy Group Specialized Register(Aug 2009),The Cochrane Central Register ofControlled Trials(The Cochrane Library Issue 2,2009),Medline(Jan 1966 to March 2010)Medline(PubMed网站2000年12月~2010年12月)和EMBASE、CBM检索文献,检索主题词:AD,treatment,guideline,self-mangement,Chinese traditional treatment,RCT;human meta analysis;systmatic review;阿尔茨海默病,治疗,指南,临床试验,中医治疗,随机对照研究试验,系统评价,Meta分析。并从真实性、重要性、实用性角度对其进行系统评价,获取最佳证据用于临床治疗。结果共检索出2篇相关临床指南,不同问题相关的随机对照试验124篇,系统评价或Meta分析14篇。对检索结果进行分析,为患者制定合理治疗方案。分析结果表明,依据该患者的病情和意愿,控制症状可以选用新型阿尔茨海默病治疗药物美金刚(memantine),其有良好的安全性,毒副作用少;目前不主张中医中药治疗;嘱咐患者家属做好监护;建议患者出院后继续用药;按时随访,同时注意药物可能的并发症。目前该患者还在随访中。结论采用循证治疗的方法,寻找老年阿尔茨海默病患者合理的治疗方案,能够提高治疗效果和临床预后。
