The aim of this study is to determine the maximal tolerance dose (MTD) and efficacy of full-dose gemcitabine and oxaliplatin when given concurrently with 3-dimentional radiation therapy (3D-RT) for locally advanced NSCLC. Methods Oxaliplatin was administered at a fixed dose of 130mg/m^2, and gemcitabine was administered at a starting dose of 800mg/m^2 with an incremental dose gradient of 200mg/m^2 for 3 dose levels. MTD was defined as the immediate dose level lower than the dose at which dose-limiting toxicity (DLT) occurred in more than one-third of the patients. The chemotherapy was administered at 3-week cycle. The RT was given as 3-D conformal manner at a single daily dose of 2Gy for 5 days per week. Results Twenty-two patients were evaluable and distributed to three different dose levels: 6 at level 1, 8 at level 2 and 8 at level 3. Pulmonary toxicity, esophageal and hematologic toxicity were the main DLT. Grade Ⅲ acute pulmonary toxicity occurred in one patient each at level 2 and level 3, both with V20>20%, and grade Ⅲ esophagitis in two patients at level 3. The MTD of gemcitabine in this study was 1000mg/m^2. The overall response rate was 75.0% (9/12). The 1- and 2-year survival rate was 70.0% and 30.5% respectively. The median time to progression was 8.7 months (range 5--11.8 months). Conclusion With reduced radiation volume, gemcitabine of 1000mg/m^2 in combination with oxaliplatin of 130mg/m^2 was effective and could be safely administered for NSCLC.展开更多
The paper proposes a decentralized concurrent transmission strategy in shared channels based on an incomplete information game in Ad Hoc networks.Based on the nodal channel quality,the game can work out a channel gain...The paper proposes a decentralized concurrent transmission strategy in shared channels based on an incomplete information game in Ad Hoc networks.Based on the nodal channel quality,the game can work out a channel gain threshold,which decides the candidates for taking part in the concurrent transmission.The utility formula is made for maximizing the overall throughput based on channel quality variation.For an achievable Bayesian Nash equilibrium(BNE) solution,this paper further prices the selfish players in utility functions for attempting to improve the channel gain one-sidedly.Accordingly,this game allows each node to distributedly decide whether to transmit concurrently with others depending on the Nash equilibrium(NE).Besides,to make the proposed game practical,this paper next presents an efficient particle swarm optimization(PSO) model to fasten the otherwise very slow convergence procedure due to the large computational complexity.Numerical results show the proposed approach is feasible to increase concurrent transmission opportunities for active nodes and the convergence can be swiftly obtained with a few of iteration times by the proposed PSO algorithm.展开更多

With the aid of the state-of-the-art modeling and simulation technology, it foresees the future or even the whole life cycle of the products designed. Osteoporosis and osteopenia are major health issues that mainly affect elderly people,women after menopause and immobilized patients.Our previous studies have proved that sclerostin antibody(Scl-Ab)can dramatically en...Osteoporosis and osteopenia are major health issues that mainly affect elderly people,women after menopause and immobilized patients.Our previous studies have proved that sclerostin antibody(Scl-Ab)can dramatically enhance bone formation and reduce bone resorption in a severe osteoporosis rat model with the combination of ovariectomy(OVX)and hindlimb immobilization(HLS).However,the mechanism in the cellular level is unclear.The objective of this study is to assess the effect of Scl-Ab on osteocytic morphology change in a combined OVX and HLS rat model via quantification of long-and short-axis and the ratio and osteocyte volume in midshaft cortical bone.Four-month-old virgin female SD rats were divided into 7 groups(n=11 per group):Sham+Veh,Sham+HLS+Veh,Sham+HLS+Scl-Ab,OVX+Veh,OVX+Scl-Ab,OVX+HLS+Veh,OVX+HLS+Scl-Ab.HLS was performed 2 weeks after sham or OVX surgery;and treatment was initiated at the time of HLS.Scl-Ab(25 mg/kg)or vehicle was subcutaneously injected twice per week for 5 weeks.Femurs were harvested at the end of study and embedded in PMMA and polished for SEM imaging.Cortical bone mid shaft osteocyte number per bone area was quantified under 1K magnification;the ratios between long axis and short axis of osteocytes were quantified under 2K magnification;osteocyte dendrite number and surface area were quantified under 5K magnification.Osteocyte dendrites width was quantified using 10K magnification.All the quantification was done by ImageJ.We have reported that multiple morphological and structural changes in osteocytes,including a decreased osteocyte density and reduced osteocyte dendrite number in HLS,OVX or the combination group and Scl-Ab’s ability to abolish these unfavorable alterations.We continued our SEM analysis on osteocytes and discovered that the oval shape of osteocyte under HLS,OVX or HLS+OVX has been distorted toward a spindle-like shape,with relatively longer long axis and shorter short axis,assuming osteocyte has a perfect spheroid shape.The ratio between long-and short-axis showed an increased trend in OVX and HLS condition,but Scl-Ab inhibited these increases(P<0.001,P<0.01,respectively).The volume decreased in HLS,OVX group,but Scl-Ab maintained osteocytes’volume in HLS condition(P<0.001).It indicates that cortical bone responds to HLS and/or OVX and Scl-Ab treatment via multiple cellular mechanisms,including density of osteocyte,dendrite number and osteocyte shape.The change of osteocyte shape may imply an altered cytoskeleton system within osteocyte and a subsequent disruption of mechanosensing ability for osteocyte,which lead to bone loss macroscopically.These data suggest Scl-Ab’s therapeutic potential could be related with its ability to maintain osteocyte’s morphologic and structural changes induced by OVX,HLS or both.展开更多

The authors present a validation concurrency control(VCC) protocol, which can enhance the performance of real-time concurrency control mechanism by reducing the number of transactions that might miss their deadlines, and compare the performance of validation concurrency control protocol with that of HP2PL(High priority two phase locking) protocol and OCC-TI-WAIT-50(Optimistic concurrency control-time interval-wait-50) protocol under shared-disk architecture by simulation. The simulation results reveal that the protocol the author presented can effectively reduce the number of transactions restarting which might miss their deadlines and performs better than HP2PL and OCC-TI-WAIT-50. It works well when arrival rate of transaction is lesser than threshold. However, due to resource contention the percentage of missing deadline increases sharply when arrival rate is greater than the threshold.展开更多
