BACKGROUND:Airway foreign bodies(AFBs)is an interdisciplinary area between emergency medicine,pediatrics and otolaryngology.It is a life-threatening condition that is not infrequently seen;however,it is poorly covered...BACKGROUND:Airway foreign bodies(AFBs)is an interdisciplinary area between emergency medicine,pediatrics and otolaryngology.It is a life-threatening condition that is not infrequently seen;however,it is poorly covered in medical literature.Accidental aspiration of an element into airways is a widespread clinical scenario among children under 3 years,predominantly males.Moreover,it is the leading cause of infantile deaths and the fourth one among preschool children.DATA RESOURCES:A systemic search was conducted in July 2015 using Pub Med/Pub Med Central Database of The National Center for Biotechnology Information(NCBI)(http://www.ncbi.nlm.nih.gov/).A total of 1 767 articles were identified and most of them were meta-analyses,systematic reviews,and case series.Those thoroughly discussing assessment and management of AFBs were retrieved.RESULTS:AFBs episodes may be either witnessed or missed.Presence of a witness for the inhalation is diagnostic.The later usually present with persistent active cough.A classical triad of paroxysmal cough,wheezing,and dyspnoea/decreased air entry was reported,though many presentations have inconsistent findings.Hence,diagnosis requires high index of clinical suspicion.Flexible fibro-optic bronchoscopy is the gold standard of diagnosis,whereas inhaled objects are best retrieved by rigid bronchoscopes.CONCLUSIONS:Close supervision of pediatrics is the hallmark of prevention.Caregivers should ensure a safe surrounding milieu,including the toys their offspring play with.Immediate complications result from direct obstruction or injury by the inhaled object.Alternatively,prolonged lodging traps air and induces inflammatory response causing atelectesis and pneumonia,respectively.展开更多
By reviewing the development history of stimulation techniques for deep/ultra-deep oil and gas reservoirs,the new progress in this field in China and abroad has been summed up,including deeper understanding on formati...By reviewing the development history of stimulation techniques for deep/ultra-deep oil and gas reservoirs,the new progress in this field in China and abroad has been summed up,including deeper understanding on formation mechanisms of fracture network in deep/ultra-deep oil and gas reservoir,performance improvement of fracturing fluid materials,fine stratification of ultra-deep vertical wells,and mature staged multi-cluster fracturing technique for ultra-deep and highly deviated wells/horizontal wells.In light of the exploration and development trend of ultra-deep oil and gas reservoirs in China,the requirements and technical difficulties in ultra-deep oil and gas reservoir stimulation are discussed:(1)The research and application of integrated geological engineering technology is difficult.(2)The requirements on fracturing materials for stimulation are high.(3)It is difficult to further improve the production in vertical profile of the ultra-deep and hugely thick reservoirs.(4)The requirements on tools and supporting high-pressure equipment on the ground for stimulation are high.(5)It is difficult to achieve efficient stimulation of ultra-deep,high-temperature and high-pressure wells.(6)It is difficult to monitor directly the reservoir stimulation and evaluate the stimulation effect accurately after stimulation.In line with the complex geological characteristics of ultra-deep oil and gas reservoirs in China,seven technical development directions are proposed:(1)To establish systematic new techniques for basic research and evaluation experiments;(2)to strengthen geological research and improve the operational mechanism of integrating geological research and engineering operation;(3)to develop high-efficiency fracturing materials for ultra-deep reservoirs;(4)to research separated layer fracturing technology for ultra-deep and hugely thick reservoirs;(5)to explore fracture-control stimulation technology for ultra-deep horizontal well;(6)to develop direct monitoring technology for hydraulic fractures in ultra-deep oil and gas reservoirs;(7)to develop downhole fracturing tools with high temperature and high pressure tolerance and supporting wellhead equipment able to withstand high pressure.展开更多

With the continuous increase in vertical depths and horizontal displacements of directional wells,the difficulties of drilling operations continue to increase,and more accurate methods of drilling difficulty evaluatio...With the continuous increase in vertical depths and horizontal displacements of directional wells,the difficulties of drilling operations continue to increase,and more accurate methods of drilling difficulty evaluation are needed.In this paper,a drilling difficulty evaluation method is built by combining drilling limit model and expert evaluation.Firstly,the concept of drilling difficulty index is introduced,and the method to calculate drilling difficulty index is established.Next,the meanings of five drilling difficulty levels are explained and the optimization design method with drilling difficulty as the target is built.At last,the theoretical model is applied to the extended-reach drilling of the Liuhua oilfield in the South China Sea,in which drilling difficulties are evaluated and the relationship between drilling difficulty and development control radius is revealed.The results indicate that extended-reach drilling in the Liuhua oilfield is on the“normal”difficulty level on average,rotary drilling in 8_(1/2)-in.section is the most difficult,and the main constraint conditions are excessive torque and high friction.Through technology upgradation,the drilling difficulties are decreased,the development control radius increases from 6.6 to 11.4 km,and the maximum horizontalto-vertical ratio increases from 5.3 to 8.7.Then,the development wells in marginal oilfields and adjustment wells in old oilfields can be drilled on“normal”difficulty level.Therefore,technology upgradation,especially drilling rig upgradation,is the most important development direction for extended-reach drilling in the South China Sea.展开更多
