结核病的荧光定量核酸扩增技术诊断及肺功能与炎性因子测定 被引量:3

目的探讨结核病的荧光定量核酸扩增技术检验及肺功能和炎性因子的变化。方法疑似结核病261例,均进行荧光定量核酸扩增技术和痰涂片检查,以影像学检查联合细菌学检查进行确诊。应用SPSS21.0软件,所获数据采用方差分析和t检验。结果确诊... 目的探讨结核病的荧光定量核酸扩增技术检验及肺功能和炎性因子的变化。方法疑似结核病261例,均进行荧光定量核酸扩增技术和痰涂片检查,以影像学检查联合细菌学检查进行确诊。应用SPSS21.0软件,所获数据采用方差分析和t检验。结果确诊结核病患者197例,占75.48%;非结核病患者64例,占24.52%。荧光定量核酸扩增技术检查结核病正确诊断197例,其中结核病诊断136例,占69.03%;阴性诊断61例,占30.96%。非结核病64例,荧光定量核酸扩增技术检查阳性2例,占3.12%;阴性62例,占96.87%。确诊结核病患者197例中痰涂片检查结核病正确诊断106例,占53.81%。阴性96例,占48.73%。非结核病64例,其中阳性5例,占7.81%,阴性59例,占92.19%。荧光定量核酸扩增技术检查结核病阳性率比较,P<0.005;肺结核阳性率比较P>0.05。荧光定量核酸扩增技术检查的灵敏度为69.04%,特异度为96.88%、约登指数0.659、阳性预测值为98.55%和阴性预测值为50.41%,均高于痰涂片检查灵敏度为51.27%、特异度为92.19%、约登指数0.435、阳性预测值为95.28%和阴性预测值为38.06%。非结核病组与结核病组比较FEV1%、FEV1/FEC%、FEF25%、FEF50%比较,P<0.0005。非结核病组与结核病组比较IL-6、IL-8、TNF-α、TGF-1比较,P<0.0005。结论荧光定量核酸扩增技术检查方法对结核病诊断具有较高价值,肺功能与炎性因子也有一定的辅助诊断作用。 展开更多

Primary tuberculosis often occurs in children with the main types of hilar lymphonode tuberculosis or Ghon complex. When pulmonary tuberculosis aggravates and tubercule bacillus invades pulmonary veins, a disseminated disease may occur clinically. One 8-year-old girl presented with hypodynamia and emaciation. CT indicated the disseminated disease and extensive mediastinal and hilar lymphadenopathy. But she had negative sputum smears and negative PPD test. The bronchi showed inflammatory change in fiberoptic bronchoscopy. Percutaneous transthoracic needle biopsy (PTNB) guided by CT was used as a means for the diagnosis of pulmonary tuberculosis. The case was rapidly diagnosed as sub-acute disseminated pulmonary tuberculosis with the help of combined CT imagine and histology. 展开更多
