Methods The clinical data of 10 patients diagnosed with UES in our hospital between 1990-2010 were extracted from hospital records and analyzed, including ophthalmologic examination, ophthalmologic ultrasonography, ultrasound biomicroscopy (UBM), fundus fluorescence angiography (FFA), indocyanine green (ICG) angiography, surgical procedures, and outcomes. Results The fundus examination of all impacted eyes showed bullous retinal detachment shifting with head position, confirmed by ultrasonography revealing retinal and choroidal detachment. UBM showed annular peripheral ciliochoroidal detachment in all cases. FFA was performed in 5 patients and revealed leopard spots without leakage from choroid into subretinal space. ICG angiograpy was performed in 3 patients and demonstrated diffused granular marked hyperfluorescence in the choroidal fluorescence in the very early phase, which increased with time and persisted until the late phase. Four eyes of 2 patients underwent full-thickness sclerectomies and 1 eye of 1 patient underwent subscleral sclerectomy, all of whom achieved reattachment of the retina without recurrence during 1-year follow-up. Conclusions Comprehensive preoperative evaluation, including ophthalmologic ultrasonography, computed tomography, and magnetic resonance imaging, is crucial for accurate classification of UES and selection of proper management strategy. Surgical treatment can achieve optimal clinical outcomes for type 1 and type 2 UES.展开更多

To determine overexpression of p53, EGFR, c-erbB-2 and c-erbB-3 in endometrioid carcinoma of the ovary and to evaluate the prognostic significance of these results, especially, coexisting overexpression of p53 and one of the member of type I growth factor receptor family.Methods. Overexpressions of the p53, EGFR, c-erbB-2 and c-erbB-3 protein were studied by im-munohistochemistry in paraffin-embedded tumor tissue from 28 patients with endometrioid carcinoma of the ovary.Results. 11 (39.3%), 13 (46. 4%), and 14 (50. 0%) were stained positively with p53, c-erbB-2 and c-erbB-3 monoclonal antibodies. 13 (46- 4%) was stained positively with EGFR polyclonal antibody. There were no relationship between p53, EGFR, C-erbB-2, c-erbB-3 and histologic grade, lymph node metastasis. The percentage of tumors with over expression of p53, EGFR, C-erbB-2 and c-erbB-3 was higher in those with stage Ⅱ-Ⅲ tumors compared with those with stage Ⅰ , in patients with residual tumor after initial surgery compared with those without. A high survival rate was observed in patients without p53, EGFR, c-erbB-2 and c-erbB-3 overexpression respectively than those with. A highest survival rate was observed in patients with both p53 and one of EGFR, c-erbB-2 and c-erbB-3 negative compared with those both positive or either of both positive.Conclusion. Overexpression of p53, EGFR, c-erbB-2 and C-erbB-3 resulted in a poorer prognosis respectively. Overexpression of both p53 and one of the EGFR, c-erbB-2 and c-erbB-3 is a worse prognostic indicator in patients with endometrioid carcinoma of the ovary.展开更多

Methods The clinical features of six such patients admitted to Peking Union Medical College Hospital during ...Objective To analyze the clinical features of hemodialysis patients complicated by infective endo carditis. Methods The clinical features of six such patients admitted to Peking Union Medical College Hospital during the year 1990 to 2009 were analyzed. All of them were diagnosed based on Chinese Children Diagnostic Criteria for Infective Endocarditis. Results The average age of the six patients was 52.3±19.3 years old. Four were males. Vascular accesses at the onset of infective endocarditis were as follows: permanent catheters in three, temporary catheters in two, and arteriovenous fistula in one. Three were fbund with mitral valve involvement, two with aor tic valve involvement, and one with both. Five vegetations were found by transthoracic echocardiography, and one by transesophageal echocardiography. Four had positive blood culture results. The catheters were all removed. Four of the patients were improved by antibiotics treatment, in which two were still on hemodialy sis in the following 14-24 months and the other two were lost to follow-up. One patient received surgery, but died of heart failure alter further hemodialysis for three months. One was well on maintenance hemodi alvsis for three months after surgery. Conclusions Infective endocarditis should be suspected when hemodialysis patients suffer from long-term fever, for which prompt blood culture and transthoracic echocardiography confirmation could be performed. Transesophageal echocardiography could be considered even when transthoracic echocardiography produces negative findings. With catheters removed, lull course of appropriate sensitive antibiotics and surgery if indicated could improve the outcome of chronic hemodialysis patients complicated by infective cndocarditis.展开更多
Objective: To study the effects of highly selective vagotomy plus resection of antral mucosa (HSV+RAM) or highly selective vagotomy (HSV) alone on the motility function of the pyloric antrum. Methods: 48 patients with...Objective: To study the effects of highly selective vagotomy plus resection of antral mucosa (HSV+RAM) or highly selective vagotomy (HSV) alone on the motility function of the pyloric antrum. Methods: 48 patients with duodenal ulcer were studied. 18 dogs were employed as experimental animals. 20 patients were operated on with HSV and 28 with HSV+RAM. The frequency of gastric evacuation and the amplitude of electrogastrography were determined 4 to 6 months after operation. 18 dogs were divided into the control group, HSV group and HSV+RAM group. The time of gastric evacuation, antral myoelectric activity and antral pressure were determined in the dogs 4 to 6 months after operation. The preoperative findings of the patients and the control dogs served as the control. Results: After operation, barium meal revealed that the shape of the stomach and duodenum was normal and the gastric peristalsis was clearly visible in human patients and experimental dogs. In the HSV+RAM group of dogs, the initial evacuation time was (5.0+0.06) min and the time of complete evacuation was (4.0+0.4) h after food-taking, which were similar to those of the control and the HSV group of dogs (P>0.05). The frequency of the antral myoelectric action potential was (3.11+0.65) cycles/min in the dog HAS+RAM group and the frequency of electrogastrography was (3.25+0.75) cycles/min in the human HSV+RAM group, which were significantly lower than those of the control and the dog and human HSV groups (P<0.05). Injection of pentagastrin in dogs and food-taking in human beings significantly increased the antral pressure and the amplitude and frequency of electrogastrography. Conclusion: The findings of this study suggest that the motility function of the reconstructed pyloric antrum in the HSV+RAM group of both the experimental dogs and human patients approaches to the normal even though there is a decrease of antral myoelectric frequency. It is suggested that HSV+RAM should be the first choice for the surgical management of duodenal ulcer.展开更多

Traumatic injuries of the penis often occur in the erected state.1 Due to the possible embarrassment, this condition has been under- reported. Typically, the patient reports a snap or cracking sound accompanied by sharp pain and rapid penile detu- mescence, followed by immediate development of swelling and angulation.2 The common causes include direct injuries, forceful masturbation, or vigorous sexual intercourse.3 Most injuries to the penis need immediate surgical treatment.
