“贵耳贱目”:新文化史视域下中国近代教学变革

明目地黄汤治疗肝肾不足型年龄相关性黄斑变性疗效及对光学相干断层扫描血管成像和视力变化的影响 被引量:4

目的探究明目地黄汤治疗对肝肾不足型年龄相关性黄斑变性患者光学相干断层扫描血管成像(optical coherence tomography angiography,OCTA)及视力变化的影响。方法采用随机数字表法将医院2020年8月—2022年8月收治的94例黄斑变性患者分... 目的探究明目地黄汤治疗对肝肾不足型年龄相关性黄斑变性患者光学相干断层扫描血管成像(optical coherence tomography angiography,OCTA)及视力变化的影响。方法采用随机数字表法将医院2020年8月—2022年8月收治的94例黄斑变性患者分为观察组与对照组各47例。对照组采用康柏西普治疗,观察组在此基础上联合使用明目地黄汤治疗。观察两组患者治疗前后使用OCTA检测中心凹层浅层的黄斑厚度及血管血流密度、中心凹旁浅层黄斑厚度及血流密度、检测脉络膜新生血管面积(area of choroidal neovascularization,CNV)、眼压、最佳矫正视力(best corrected visual acuity,BCVA)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、C反应蛋白(C-reactive protein,CRP)。观察两组患者临床疗效与并发症发生情况。结果两组患者治疗前CNV、黄斑中心凹厚度、黄斑中心凹旁厚度、黄斑中心凹血流密度和黄斑中心凹旁血流密度差异无统计学意义,治疗后观察组与对照组黄斑中心凹旁血流密度差异无统计学意义(P>0.05)。治疗后观察组CNV、黄斑中心凹厚度、黄斑中心凹旁厚度水平和黄斑中心凹血流密度较对照组显著降低(P<0.05)。两组患者治疗前眼压和BCVA水平差异无统计学意义,治疗后两组眼压也差异无统计学意义(P>0.05)。治疗后观察组BCVA水平显著低于对照组(P<0.05)。两组患者治疗前VEGF、CRP水平差异无统计学意义(P>0.05)。治疗后观察组VEGF、CRP水平显著低于对照组(P<0.05)。观察组总有效率为91.49%(43/47)显著高于对照组74.47%(35/47)(χ^(2)=4.821,P=0.028)。观察组并发症发生率为14.89%(7/47)略高于对照组8.52%(4/47),但两组并发症发生率差异无统计学意义(χ^(2)=0.927,P=0.336)。结论明目地黄汤能有效降低炎症因子的表达、改善眼底出血和新生血管眼底情况、利于视力恢复,改善OCTA的表现。 展开更多

目的探讨白内障术后干眼症(DED)患者采用润目方熏蒸联合地夸磷索钠滴眼液治疗对其临床疗效、泪液分泌质量、中医症状、生命质量等的改善情况。方法将苏州中西医结合医院眼科2021年10月—2022年10月收治的白内障术后DED患者80例作为此次... 目的探讨白内障术后干眼症(DED)患者采用润目方熏蒸联合地夸磷索钠滴眼液治疗对其临床疗效、泪液分泌质量、中医症状、生命质量等的改善情况。方法将苏州中西医结合医院眼科2021年10月—2022年10月收治的白内障术后DED患者80例作为此次研究的研究对象,按照就诊先后顺序并以随机化原则分为对照组(37例)和观察组(43例)。两组于术后均给予妥布霉素地塞米松滴眼液,此基础上对照组常规给予地夸磷索钠滴眼液治疗,观察组在前述对照组用药基础上加用润目方熏蒸,均治疗2疗程。比较两组治疗2疗程后的临床疗效,治疗前及治疗1、2疗程后的泪液分泌质量,治疗前及治疗2疗程后的中医症状积分、美国国家眼科研究所视觉相关生命质量量表(NEI-VFQ-25)评分,治疗其间不良反应发生情况。结果对照组治疗2疗程的总有效率(62.16%)相比观察组(86.05%)更低(P<0.05)。治疗1、2疗程后相比于治疗前,两组泪膜破裂时间(BUT)逐渐延长,观察组均长于对照组,基础泪液分泌试验(SITI)长度逐渐增长,观察组均长于对照组,角膜荧光素染色(FL)评分逐渐降低,观察组均低于对照组(P<0.05)。治疗2疗程后相比于治疗前,两组中医症状积分各项分值均降低,观察组低于对照组,NEI-VFQ-25评分各项的分值均升高,观察组高于对照组(P<0.05)。治疗期间的总不良反应发生率两组相比差异无统计学意义(P>0.05)。结论润目方熏蒸联合地夸磷索钠滴眼液应用于白内障术后DED患者可改善泪液分泌质量,减轻临床症状,提升生命质量,具有较好的临床效果,且安全性良好,临床应用疗效确切。 展开更多

目的 观察止血祛瘀明目片联合康柏西普眼内注射对糖尿病性白内障术后黄斑水肿(macular edema, DME)的影响。方法 选取该院自2019年4月—2021年12月收治行白内障超声乳化吸除人工晶状体植入术治疗后出现DME的糖尿病患者104例(单眼患病),... 目的 观察止血祛瘀明目片联合康柏西普眼内注射对糖尿病性白内障术后黄斑水肿(macular edema, DME)的影响。方法 选取该院自2019年4月—2021年12月收治行白内障超声乳化吸除人工晶状体植入术治疗后出现DME的糖尿病患者104例(单眼患病),根据随机数字法分为两组。对照组给予康柏西普眼内注射,而观察组加用止血祛瘀明目片治疗。比较两组最佳矫正视力(best-corrected visual acuity, BCVA)、眼压、黄斑中心凹视网膜厚度(central macular thickness, CMT)、血清相关因子及证候积分的差异。结果 基线时,两组DME患者BCVA、眼压、CMT比较,差异无统计学意义(P>0.05)。治疗1、3个月后,两组DME患者BCVA较基线升高,而眼压、CMT则明显下降,且观察组DME患者BCVA升高和眼压、CMT下降幅度均大于对照组,差异有统计学意义(P<0.05)。基线时,两组DME患者血清相关因子、证候积分比较,差异无统计学意义(P>0.05)。治疗3个月后,两组DME患者干扰素诱导蛋白-10(IP-10)、白细胞介素-6(IL-6)、巨噬细胞趋化蛋白-1(MCP-1)及视物不清、目睛干涩、头晕耳鸣、口咽干燥等证候积分均较基线下降,且观察组DME患者血清相关因子、证候积分下降幅度均大于对照组,差异有统计学意义(P<0.05)。观察组发生皮疹1例,对照组无相关不良反应发生。结论 止血祛瘀明目片联合联合康柏西普眼内注射治疗糖尿病性白内障术后DME可降低眼压,矫正视力,减轻症状和黄斑水肿程度,可能与调节相关血清因子有关。 展开更多

目的建立Graves眼病动物模型,观察平目汤对造模雌性BALB/c小鼠眼眶脂肪积聚的影响,并探讨其作用的分子机制和发挥作用可能的靶点。方法通过使用重组h-TSHR质粒基因免疫结合电穿孔的方法,建立Graves眼病动物模型。使用平目汤不同剂量及... 目的建立Graves眼病动物模型,观察平目汤对造模雌性BALB/c小鼠眼眶脂肪积聚的影响,并探讨其作用的分子机制和发挥作用可能的靶点。方法通过使用重组h-TSHR质粒基因免疫结合电穿孔的方法,建立Graves眼病动物模型。使用平目汤不同剂量及地塞米松灌胃4周,观察小鼠眼球后脂肪组织占眼球重量百分比、血清TRAb、甲状腺与眼球组织病理。结果(1)平目汤不同剂量及地塞米松干预造模小鼠后,小鼠眼球后脂肪组织占眼球重量百分比由低到高依次为:空白组<平目汤中剂量(PMT-M)组<平目汤低剂量(PMT-L)组<地塞米松组<平目汤高剂量(PMT-H)组<模型组。平目汤不同剂量组与地塞米松组均较模型组下降(P<0.05),PMT-L组与PMT-M组较地塞米松组和PMT-H组下降更加显著(P<0.05)。(2)小鼠血清TRAb水平由低到高依次为:空白组<PMT-M组<PMT-L组<地塞米松组<PMT-H组<模型组。与空白组相比,除PMT-M组外其他组别有统计学意义(P<0.05);与模型组相比,平目汤不同剂量与地塞米松干预后TRAb明显下降(P<0.05);与地塞米松组相比,PMT-L组与PMT-M组下降显著(P<0.05),其中PMT-M组更加明显。(3)眼球组织HE染色:与模型组相比,平目汤各组眼球后脂肪组织堆积减少,地塞米松组无明显改变。(4)甲状腺组织HE染色:模型组可见甲状腺淋巴细胞浸润,间质增生明显。平目汤不同剂量干预后淋巴细胞浸润减少,地塞米松干预后甲状腺少量淋巴细胞浸润。结论平目汤通过有效抑制Graves眼病小鼠眼球后脂肪组织的积聚而发挥疗效。 展开更多

目的探究和血明目片结合雷珠单抗治疗视网膜静脉阻塞合并黄斑水肿临床疗效及安全性。方法研究合计纳入92例视网膜静脉阻塞合并黄斑水肿患者,均由医院门诊部2019年1月—2019年12月收治,采取随机数字表法将患者分为观察组及对照组,对照组... 目的探究和血明目片结合雷珠单抗治疗视网膜静脉阻塞合并黄斑水肿临床疗效及安全性。方法研究合计纳入92例视网膜静脉阻塞合并黄斑水肿患者,均由医院门诊部2019年1月—2019年12月收治,采取随机数字表法将患者分为观察组及对照组,对照组患者(46例)采取雷珠单抗治疗,观察组患者(46例)采取和血明目片结合雷珠单抗治疗,比较两组患者临床疗效、治疗前后症状及体征(视物模糊、眼前黑影遮挡、视物变形、黄斑水肿、视网膜出血等)积分变化、视网膜循环时间变化、治疗后视网膜出血情况、治疗前后血液流变学指标变化、黄斑中心凹视网膜厚度变化及视力变化、不良反应。结果观察组患者治疗总有效率(93.48%,43/46)高于对照组(80.43%,37/46),P<0.05;治疗前,两组患者视物模糊、眼前黑影遮挡、视物变形、黄斑水肿、视网膜出血等症状及体征积分、视网膜循环时间、黄斑中心凹视网膜厚度、视力等指标比较,P>0.05,治疗后各组患者视物模糊、眼前黑影遮挡、视物变形、黄斑水肿、视网膜出血等症状及体征积分、视网膜循环时间、黄斑中心凹视网膜厚度、视力等指标改善,观察组治疗后视物模糊、眼前黑影遮挡、视物变形、黄斑水肿、视网膜出血等症状及体征积分、视网膜循环时间、黄斑中心凹视网膜厚度、视力等指标均优于对照组(P<0.05);观察组患者治疗后视网膜出血情况改善,优于对照组,P<0.05;治疗前,两组患者纤维蛋白原、全血高切黏度及全血低切黏度比较,P>0.05,治疗后两组患者纤维蛋白原、全血高切黏度及全血低切黏度指标均下降(P<0.05),治疗后两组纤维蛋白原、全血高切黏度指标差异无统计学意义(P>0.05),而全血低切黏度指标优于对照组(P<0.05);对照组与观察组不良反应率较低,均为4.35%(P>0.05)。结论和血明目片结合雷珠单抗治疗视网膜静脉阻塞合并黄斑水肿效果显著,患者症状及体征改善,视力恢复较好,且不良反应少,安全性高,值得应用。 展开更多

The YOLOv8 model faces challenges with dense target distribution and small size,resulting in lower accuracy in dense small target detection.To address these issues,an improved small target detection algorithm based on the YOLOv8 model was proposed in this paper.Firstly,the Global Attention Module(GAM)was introduced to enhance data prediction capability and model expression ability.Secondly,the Space-to-Depth(SPD)module was incorporated into the backbone network for fine-grained feature information learning to mitigate feature information loss due to down-sampling.Finally,a 160 pixels×160 pixels feature layer was added to expand small target feature information and effectively reduce instances of missed targets.Experimental validation on the public VisDrone2019 UAV small target detaset demonstrated that the proposed model achieves significant performance improvement in small target detection tasks compared to existing models,exhibiting higher accuracy. 展开更多

Lunar impact crater detection is crucial for lunar surface studies and spacecraft landing missions,yet deep learning still struggles with accurately detecting small craters,especially when relying on incomplete catalogs.In this work,we integrate Digital Elevation Model(DEM)data to construct a high-quality dataset enriched with slope information,enabling a detailed analysis of crater features and effectively improving detection performance in complex terrains and low-contrast areas.Based on this foundation,we propose a novel two-stage detection network,MSFNet,which leverages multi-scale adaptive feature fusion and multisize ROI pooling to enhance the recognition of craters across various scales.Experimental results demonstrate that MSFNet achieves an F1 score of 74.8%on Test Region1 and a recall rate of 87%for craters with diameters larger than 2 km.Moreover,it shows exceptional performance in detecting sub-kilometer craters by successfully identifying a large number of high-confidence,previously unlabeled targets with a low false detection rate confirmed through manual review.This approach offers an efficient and reliable deep learning solution for lunar impact crater detection. 展开更多

In recent years,with the development of synthetic aperture radar(SAR)technology and the widespread application of deep learning,lightweight detection of SAR images has emerged as a research direction.The ultimate goal is to reduce computational and storage requirements while ensuring detection accuracy and reliability,making it an ideal choice for achieving rapid response and efficient processing.In this regard,a lightweight SAR ship target detection algorithm based on YOLOv8 was proposed in this study.Firstly,the C2f-Sc module was designed by fusing the C2f in the backbone network with the ScConv to reduce spatial redundancy and channel redundancy between features in convolutional neural networks.At the same time,the Ghost module was introduced into the neck network to effectively reduce model parameters and computational complexity.A relatively lightweight EMA attention mechanism was added to the neck network to promote the effective fusion of features at different levels.Experimental results showed that the Parameters and GFLOPs of the improved model are reduced by 8.5%and 7.0%when mAP@0.5 and mAP@0.5:0.95 are increased by 0.7%and 1.8%,respectively.It makes the model lightweight and improves the detection accuracy,which has certain application value. 展开更多

Detection of geohazards caused by human disturbance activities based on convolutional neural networks

Human disturbance activities is one of the main reasons for inducing geohazards.Ecological impact assessment metrics of roads are inconsistent criteria and multiple.From the perspective of visual observation,the environment damage can be shown through detecting the uncovered area of vegetation in the images along road.To realize this,an end-to-end environment damage detection model based on convolutional neural network is proposed.A 50-layer residual network is used to extract feature map.The initial parameters are optimized by transfer learning.An example is shown by this method.The dataset including cliff and landslide damage are collected by us along road in Shennongjia national forest park.Results show 0.4703 average precision(AP)rating for cliff damage and 0.4809 average precision(AP)rating for landslide damage.Compared with YOLOv3,our model shows a better accuracy in cliff and landslide detection although a certain amount of speed is sacrificed. 展开更多

Stegolophodon is an age-informative genus of mammals that had a widespread distribution during the Neogene.This paper reports the discovery of Stegolophodon fossils from the Middle Miocene lower Fotan Formation at the Zhangpu locality,Fujian Province,China.This discovery represents the first evidence of Neogene proboscidean fossils in southeastern China.The newly found molar materials have low tooth crowns,very straight lophs/lophids,and an indistinct median sulcus.The mesoconelets and posterior cingulum are well-developed,while the second posterior pretrite central conule is significantly reduced.These specimens closely resemble Stegolophodon pseudolatidens in cheek tooth morphology,and can thus be attributed to the same species.This discovery fills a gap in the fossil record of large mammals in this region during the Neogene and provides valuable insights into the evolution of proboscideans and paleoenvironments. 展开更多

This paper presents an investigation on the target-guided coordinated control(TACC)of unmanned surface vehicles(USVs).In the scenario of tracking non-cooperative targets,the status information of the target can only be obtained by some USVs.In order to achieve semi-encirclement tracking of noncooperative targets under maritime security conditions,a fixed-time tracking control method based on dynamic surface control(DSC)is proposed in this paper.Firstly,a novel TACC architecture with decoupled kinematic control law and decoupled kinetic control law was designed to reduce the complexity of control system design.Secondly,the proposed DSC-based target-guided kinematic control law including tracking points pre-allocation strategy and sigmoid artificial potential functions(SigAPFs)can avoid collisions during tracking process and optimize kinematic control output.Finally,a fixed-time TACC system was proposed to achieve fast convergence of kinematic and kinetics errors.The effectiveness of the proposed TACC approach in improving target tracking safety and reducing control output chattering was verified by simulation comparison results. 展开更多

In response to the scarcity of infrared aircraft samples and the tendency of traditional deep learning to overfit,a few-shot infrared aircraft classification method based on cross-correlation networks is proposed.This method combines two core modules:a simple parameter-free self-attention and cross-attention.By analyzing the self-correlation and cross-correlation between support images and query images,it achieves effective classification of infrared aircraft under few-shot conditions.The proposed cross-correlation network integrates these two modules and is trained in an end-to-end manner.The simple parameter-free self-attention is responsible for extracting the internal structure of the image while the cross-attention can calculate the cross-correlation between images further extracting and fusing the features between images.Compared with existing few-shot infrared target classification models,this model focuses on the geometric structure and thermal texture information of infrared images by modeling the semantic relevance between the features of the support set and query set,thus better attending to the target objects.Experimental results show that this method outperforms existing infrared aircraft classification methods in various classification tasks,with the highest classification accuracy improvement exceeding 3%.In addition,ablation experiments and comparative experiments also prove the effectiveness of the method. 展开更多

To address the challenges of low detection accuracy caused by the diverse species,significant size variations,and complex growth environments of wheat pests in natural settings,a PSA-YOLO11n algorithm is proposed to enhance detection precision.Building upon the YOLO11n framework,the proposed improvements include three key components:1)SimCSPSPPF in Backbone:An improved Spatial Pyramid Pooling-Fast(SPPF)module,SimCSPSPPF,is integrated into the Backbone to reduce the number of channels in the hidden layers,thereby accelerating model training.2)PEC in Neck:The standard convolution layers in the Neck are replaced with Perception Enhancement Convolutions(PEC)to improve multi-scale feature extraction capabilities,enhancing detection speed.3)AWIoU Loss Function:The regression loss function is replaced with Adequate Wise IoU(AWIoU),addressing issues of bounding box distortion caused by the diversity in pest species and size variations,thereby improving the precision of bounding box localization.Experimental evaluations on the IP102 dataset demonstrate that PSA-YOLO11n achieves a mean Average Precision(mAP)of 89.10%,surpassing YOLO11n by 0.8%.Comparisons with other mainstream algorithms,including Faster R-CNN,RetinaNet,YOLOv5s,YOLOv8n,YOLOv10n,and YOLO11n,confirm that PSA-YOLO11n outperforms all baselines in terms of detection performance.These results highlight the algorithm’s capability to significantly improve the detection accuracy of multi-scale wheat pests in natural environments,providing an effective solution for pest management in wheat production. 展开更多

Gear assembly errors can lead to the increase of vibration and noise of the system,which affect the stability of system.The influence can be compensated by tooth modification.Firstly,an improved three-dimensional loaded tooth contact analysis(3D-LTCA)method which can consider tooth modification and coupling assembly errors is proposed,and mesh stiffness calculated by proposed method is verified by MASTA software.Secondly,based on neural network,the surrogate model(SM)that maps the relationship between modification parameters and mesh mechanical parameters is established,and its accuracy is verified.Finally,SM is introduced to establish an optimization model with the target of minimizing mesh stiffness variations and obtaining more even load distribution on mesh surface.The results show that even considering training time,the efficiency of gear pair optimization by surrogate model is still much higher than that by LTCA method.After optimization,the mesh stiffness fluctuation of gear pair with coupling assembly error is reduced by 34.10%,and difference in average contact stresses between left and right regions of the mesh surface is reduced by 62.84%. 展开更多
