Diet restriction and exercise alleviate cognitive reduction of high fat diet (HFD)-induced obese mice by rescuing inflammation-mediated compromised insulin signaling pathway through activating AMPK/SIRT1 signal pathway and suppressing TLR4 signal pathway

Obesity,caused by excessive energy,leads to body weight gain and various diseases,including cognitive impairment.Current studies suggest that diet restriction such as optimal fasting and regular exercise are crucial for improving cognitive capacity.However,further exploration is needed to understand the specific mechanisms of high fat diet(HFD)-induced cognitive decline in obesity.In the present study,4-month-old mice were subjected to HFD feeding for 18 weeks,followed by aerobic exercise and high-intensity intermittent exercise,regular diet feeding,and intermittent fasting for 8 weeks,and then used to evaluate cognitive capacity,inflammation,compromised insulin signaling pathway,and apoptosis in hippocampal tissue,as well as AMPK/SIRT1 and TLR4 signal pathways.Obese mice revealed impaired cognitive capacity as compared with mice fed with regular diets.In contrast,aerobic exercise,high-intensity intermittent exercise,regular diet,and intermittent fasting could inhibit apoptosis caused by inflammation-mediated compromised insulin signaling pathway in hippocampal tissues through activating the AMPK/SIRT1 signal pathway and suppressing the TLR4 signal pathway,thereby rescuing the cognitive impairment of obese mice.Therefore,diet restriction and exercise interventions may play a positive role in reverting obesity-induced cognitive impairment. 展开更多

The exercise recommendation system is emerging as a promising application in online learning scenarios,providing personalized recommendations to assist students with explicit learning directions.Existing solutions generally follow a collaborative filtering paradigm,while the implicit connections between students(exercises)have been largely ignored.In this study,we aim to propose an exercise recommendation paradigm that can reveal the latent connections between student-student(exercise-exercise).Specifically,a new framework was proposed,namely personalized exercise recommendation with student and exercise portraits(PERP).It consists of three sequential and interdependent modules:Collaborative student exercise graph(CSEG)construction,joint random walk,and recommendation list optimization.Technically,CSEG is created as a unified heterogeneous graph with students’response behaviors and student(exercise)relationships.Then,a joint random walk to take full advantage of the spectral properties of nearly uncoupled Markov chains is performed on CSEG,which allows for full exploration of both similar exercises that students have finished and connections between students(exercises)with similar portraits.Finally,we propose to optimize the recommendation list to obtain different exercise suggestions.After analyses of two public datasets,the results demonstrated that PERP can satisfy novelty,accuracy,and diversity. 展开更多

Exercise Fatigue Monitoring Based on R-Peak Detection Using UNET-TCN

It is pressing for a device that can detect the intensity of exercise.Therefore, in order to enable real-time detection of exercise intensity and mitigate the risks of harmfrom excessive exercise, a exercise intensity monitoring system based on the heart rate variability(HRV) from electrocardiogram (ECG) signal and linear features from phonocardiogram (PCG)signal is proposed. The main contributions include: First, accurate analysis of HRV is crucial forsubsequent exercise intensity detection. To enhance HRV analysis, we propose an R-peak detectorbased on encoder-decoder and temporal convolutional network (TCN). Experimental resultsdemonstrate that the proposed R-peak detector achieves an F1 score exceeding 0.99 on real high-intensity exercise ECG datasets. Second, an exercise fatigue monitoring system based on multi-signal feature fusion is proposed. Initially, utilizing the proposed R-peak detector for HRV extractionin exercise intensity detection,which outperforms traditional algorithms, with the system achieving a classification performance of 0.933 sensitivity, 0.802 specificity, and 0.960 accuracy. To further improve the system, we combine HRV with the linear features of PCG. Our exercise intensitydetection system achieves 90.2% specificity, 96.7% recall, and 98.1% accuracy in five-fold cross-validation. 展开更多

Accurate detection of exercise fatigue based on physiological signals is vital for reason-able physical activity.As a non-invasive technology,phonocardiogram(PCG)signals possess arobust capability to reflect cardiovascular information,and their data acquisition devices are quiteconvenient.In this study,a novel hybrid approach of fractional Fourier transform(FRFT)com-bined with linear and discrete wavelet transform(DWT)features extracted from PCG is proposedfor PCG multi-class classification.The proposed system enhances the fatigue detection performanceby combining optimized FRFT features with an effective aggregation of linear features and DWTfeatures.The FRFT technique is employed to convert the 1-D PCG signal into 2-D image which issent to a pre-trained convolutional neural network structure,called VGG-16.The features from theVGG-16 were concatenated with the linear and DWT features to form fused features.The fusedfeatures are sent to support vector machine(SVM)to distinguish six distinct fatigue levels.Experi-mental results demonstrate that the proposed fused features outperform other feature combinationssignificantly. 展开更多

Coronary heart disease (CHD) is the leading cause of death worldwide and becomes increasingly prevalent among patients aged 65 years and older.Elderly patients are at a higher risk for complications and accelerated physical deconditioning after a cardiovascular event,especially compared to their younger counterparts.The last few decades were privy to multiple studies that demonstrated the beneficial effects of cardiac rehabilitation (CR) and exercise therapy on mortality,exercise capacity,psychological risk factors,inflammation,and obesity among patients with CHD.Unfortunately,a significant portion of the available data in this field pertains to younger patients.A viable explanation is that older patients are grossly underrepresented in these programs for multiple reasons starting with the patient and extending to the physician.In this article,we will review the benefits of CR programs among the elderly,as well as some of the barriers that hinder their participation. 展开更多

Heart failure (HF) with preserved ejection fraction (HFpEF) is the most common form of HF in older adults, and is increasing in preva- lence as the population ages. Furthermore, HFpEF is increasing out of proportion to HF with reduced EF (HFrEF), and its prognosis is worsening while that of HFrEF is improving. Despite the importance of HFpEF, our understanding of its pathophysiology is incomplete, and optimal treatment remains largely undefined. A cardinal feature of HFpEF is reduced exercise tolerance, which correlates with symptoms as well as reduced quality of life. The traditional concepts of exercise limitations have focused on central dysfimction related to poor cardiac pump function. However, the mechanisms are not exclusive to the heart and lungs, and the understanding of the pathophysiology of this dis- ease has evolved. Substantial attention has focused on defining the central versus peripheral mechanisms underlying the reduced functional capacity and exercise tolerance among patients with HF. In fact, physical training can improve exercise tolerance via peripheral adaptive mechanisms even in the absence of favorable central hemodynamic function. In addition, the drug trials performed to date in HFpEF that have focused on influencing cardiovascular function have not improved exercise capacity. This suggests that peripheral limitations may play a significant role in HF limiting exercise tolerance, a hallmark feature of HFpEF. 展开更多

Background: Excessive elevation of arterial blood pressure(BP) at high altitude can be detrimental to our health due to acute mountain sickness(AMS) or some AMS symptoms. This prospective and observational study aimed to elucidate blood pressure changes induced by exposure to high-altitude hypoxia and the relationships of these changes with AMS prevalence, AMS severity, sleep quality and exercise condition in healthy young men.Methods: A prospective observational study was performed in 931 male young adults exposed to high altitude at 3,700 m(Lhasa) from low altitude(LA, 500 m). Blood pressure measurement and AMS symptom questionnaires were performed at LA and on day 1, 3, 5, and 7 of exposure to high altitude. Lake Louise criteria were used to diagnose AMS. Likewise, the Athens Insomnia Scale(AIS) and the Epworth Sleepiness Scale(ESS) were filled out at LA and on day 1, 3, and 7 of exposure to high altitude.Results: After acute exposure to 3,700 m, diastolic blood pressure(DBP) and mean arterial blood pressure(MABP) rose gradually and continually(P【0.05). Analysis showed a relationship with AMS for only MABP(P【0.05) but not for SBP and DBP(P】0.05). Poor sleeping quality was generally associated with higher SBP or DBP at high altitude, although inconsistent results were obtained at different time(P【0.05). SBP and Pulse BP increased noticeably after high-altitude exercise(P【0.05).Conclusions: Our data demonstrate notable blood pressure changes under exposure to different high-altitude conditions: 1) BP increased over time. 2) Higher BP generally accompanied poor sleeping quality and higher incidence of AMS. 3) SBP and Pulse BP were higher after high-altitude exercise. Therefore, we should put more effort into monitoring BP after exposure to high altitude in order to guard against excessive increases in BP. 展开更多

Background:Acute mountain sickness(AMS)is the mildest form of acute altitude illnesses,and consists of nonspecific symptoms when unacclimatized persons ascend to elevation of≥2500 m.Risk factors of AMS include:the altitude,individual susceptibility,ascending rate and degree of pre-acclimatization.In the current study,we examined whether physiological response at low altitude could predict the development of AMS.Methods:A total of 111 healthy adult healthy volunteers participated in this trial;and 99(67 men and 32 women)completed the entire study protocol.Subjects were asked to complete a 9-min exercise program using a mechanically braked bicycle ergometer at low altitude(500 m).Heart rate,blood pressure(BP)and pulse oxygen saturation(SpO2)were recorded prior to and during the last minute of exercise.The ascent from 500 m to 4100 m was completed in 2 days.AMS was defined as≥3 points in a 4-item Lake Louise Score,with at least one point from headache wat 6–8 h after the ascent.Results:Among the 99 assessable subjects,47(23 men and 24 women)developed AMS at 4100 m.In comparison to the subjects without AMS,those who developed AMS had lower proportion of men(48.9%vs.84.6%,P<0.001),height(168.4±5.9 cm vs.171.3±6.1 cm,P=0.019),weight(62.0±10.0 kg vs.66.7±8.6 kg,P=0.014)and proportion of smokers(23.4%vs.51.9%,P=0.004).Multivariate regression analysis revealed the following independent risks for AMS:female sex(odds ratio(OR)=6.32,P<0.001),SpO2 change upon exercise at low altitude(OR=0.63,P=0.002)and systolic BP change after the ascent(OR=0.96,P=0.029).Women had larger reduction in SpO2 after the ascent,higher AMS percentage and absolute AMS score.Larger reduction of SpO2 after exercise was associated with both AMS incidence(P=0.001)and AMS score(P<0.001)in men but not in women.Conclusions:Larger SpO2 reduction after exercise at low altitude was an independent risk for AMS upon ascent.Such an association was more robust in men than in women.Trial registration:Chinese Clinical Trial Registration,ChiCTR1900025728.Registered 6 September 2019. 展开更多

Nevertheless, there is no val- idated Chinese version. The aim of this study is to determine whether a questionnaire-based method using the Chinese version of VSAQ (the modified VSAQ) is a practical tool to assess exercise tolerance of Chinese elderly with coronary heart disease. Methods One hundred thirty consecutive elderly patients who were diagnosed with coronary heart disease (CHD) (mean age 68.9 -4- 6.0 years) referred for treadmill exercise testing (TET) for clinical reasons were included in the study. They were asked to complete a questionnaire for clinical characteristics information on age, sex, history, exercise habits, medications, the original VSAQ and the modified VSAQ. We investigated the relationship between exer- cise tolerance in metabolic equivalents (METs) estimated by VSAQ and that obtained by TET. Results The METs by the original VSAQ and the modified VSAQ did not difference significantly (P = 0.528). The modified VSAQ scores were significantly correlated with the METs oh- mined by TET (r = 0.819, 95% CI: 0.7534).873, P 〈 0.01), and the scores of original VSAQ also correlated with the METs by TET (r = 0.804, 95% CI: 0.7454).854, P 〈 0.01). The Bland-Altman graph analysis showed few values outside the limits of agrcement, suggesting good precision between the METs estimated by questionnaire and the METs obtained by TET. Conclusions The Chinese version of the VSAQ confirmed its validity and equivalence to the original version, especially when evaluating individuals with coronary heart disease and older adults. The results showed that the VSAQ is a valuable tool to assess the exercise tolerance. 展开更多

Effects of Exercise on Cancer-related Fatigue and Quality of Life in Prostate Cancer Patients Undergoing Androgen Deprivation Therapy: A Meta-analysis of Randomized Clinical Trials 被引量:2

Objective To gain insight on how exercise affects the outcomes of prostate cancer patients treated with androgen deprivation therapy,specifically cancer-related fatigue(CRF) and quality of life(QoL). Methods Systematic searches for randomized clinical trials(RCTs) evaluating the effects of exercise on CRF and QoL of prostate cancer patients receiving androgen deprivation therapy were carried out to identify the eligible studies from EMBASE,Pub Med and Cochrane library. Related data were extracted from eligible studies and then subjected to Reviewer Manage 5.3 for analysis. Standardized mean differences(SMD) and its 95% confidence interval(CI) were calculated. Results In all,10 RCTs involving 841 prostate cancer patients(448 of whom exercised and 393 did not) were included in this study. With respect to CRF,there was good consistency among different studies,and it was remarkably reduced in the exercise group(SMD=-0.32,95% CI:-0.45 to-0.18,P<0.00001,n=784). In regards to QoL,there was also good consistency among different studies,and it was also improved significantly in the exercise group(SMD=0.21,95% CI:0.08 to 0.34,P=0.002,n=841). Conclusion Exercise both reduced CRF and improved QoL in prostate cancer patients receiving androgen deprivation therapy. 展开更多

Correlation between exercise, personal income level and health-related quality of life in patients with newly diagnosed stable angina 被引量:2

Background:Stable angina is one of the most common clinical types of coronary artery disease(CAD)and associated with poor health-related quality of life(HRQL).However,few studies have evaluated the risk factors associated with HRQL in patients with newly diagnosed stable angina.Methods:A cross-sectional survey was conducted with 342 consecutive outpatients with newly diagnosed stable angina from October 2017 to January 2018 at the Second Affiliated Hospital of Army Medical University,Chongqing,China.Eight dimensions of HRQL were evaluated via the 36-item Short-Form Health Survey,including physical functioning,role limitation due to physical problems,bodily pain,general health,vitality,social functioning,role limitation due to emotional problems,and mental health.Physical and mental component summaries were calculated.Multiple stepwise regression was performed to determine the factors associated with HRQL.Results:Patients who were older,were females,did not exercise,had lower educational levels,had lower monthly incomes,had smoking/drinking habits,and had diabetes,hypertension,or hyperlipemia showed lower physical HRQL scores,while those who were older with lower educational levels and lower monthly incomes showed lower mental HRQL scores.The results of the multiple stepwise regression analyses showed that physical and mental HRQL were positively correlated with exercise and monthly income and negatively associated with age.Patients with monthly income≥5000 Yuan showed higher HRQL scores than those with monthly income<5000 Yuan.Sleep quality and drinking were negatively associated with physical,but not mental HRQL.Conclusions:Our findings indicated that exercise and personal income level,both modifiable factors,were positively associated with physical and mental HRQL.These findings could have implications for clinical suggestions and strategies to improve HRQL in patients with stable angina. 展开更多

Objective To study the differences of cardiovascular system between men and women in response to exercise stress. Methods Forty healthy youth were tested according to Bruce protocol of exercise stress. They were detected by ultrasonography during the rest, peak exercise, and recovery stages, respectively. The left ventricular diastolic elastance(Ed), effective arterial elastance(Ea), left ventricular end-systolic elatance(Ees), ventricular-vascular coupling index(VVI), and total stiffness index(TSI) were measured and calculated according to the formulas. The results of all stages were compared according to genders. Results All stages, the Ed, TSI, and VVI of women were higher than those of men, but the Ees was lower than that of men(all P<0.05); there was no significant difference in Ea between men and women. The Ed, Ees, Ea, and TSI were closely related with left ventricular oxygen consumption and heart function, and women showed more closely. Before and after exercise, the changes were different in Ed, Ees, Ea, TSI, and VVI(all P<0.05), and VVI changed least. Conclusions Before and after exercise, the ventricular stiffness matched well with arterial stiffness and maintained within a narrow range. For women, the tolerance of exercise was lower than that of men. 展开更多

Background Elevated left ventricular filling pressure (LVFP) is an important cause of exercise intolerance in patients with atrial fib- dilation (AF). Exercise stress echocardiography could assess LVFP during exercise. The objective of this study was to investigate the relationship between exercise induced elevation of LVFP and exercise capacity in patients with AF. Methods This study included 145 con- secutive patients (81 men and 64 women; mean age 65.5 ± 8.0 years) with persistent non-valvular AF and normal left ventricular systolic function (left ventdcular ejection fraction 〉 50%). All patients underwent a symptom-limited cardiopulmonary exercise test (CPET). Doppler echocardiography was performed both at rest and immediately after exercise. Five consecutive measurements of early diastolic mitral inflow velocity (E) and early diastolic mitral annular velocity (e') were taken and averaged. E/e' ratio was calculated. Elevated LVFP was defined as E/e' 〉 9, and patients with elevated LVFP at rest were excluded. Results Patients were classified into two groups according to LVFP estimated by E/e' ratio after exercise: 39 (26.9%) with elevated LVFP after exercise and 106 (73.1%) with normal LVFP. As compared with patients with normal LVFP, the ones with elevated LVFP after exercise had significantly lower peak oxygen uptake (VO2 peak) (21.7 ± 2.3 vs. 26.4 ± 3.8 mL/min per kilogram, P 〈 0.001), lower anaerobic threshold (19.9 ± 2.5 vs. 26.0± 4.0 mL/min per kilogram, P 〈 0.001), and shorter exercise time duration (6.2± 0.8 vs. 7.0 ±1.3 min, P 〈 0.001). Multivariate analysis showed that age, gender and E/e' after exercise were significantly correlated with VO2peak. Conclusion Elevated LVFP estimated by E/e' ratio after exercise is independently associated with reduced exercise capacity in AF patients. 展开更多

Cardiac response to exercise in mild-to-moderate chronic obstructive pulmonary disease

Objective Chronic obstructive pulmonary disease (COPD) increases the risk of cardiovascular problem.The symptom of dyspnea on exertion may be associated with pulmonary dysfunction or heart failure, or both. The study objective was to determine whether cardiac dysfunction adds to the mechanism of dyspnea caused mainly by impaired lung function in patients with mild-tomoderate COPD. Methods Patients with COPD and healthy controls performed incremental and constant work rate exercise testing. Venous blood samples were collected in 19 COPD patients and 10 controls before and during constant work exercise for analysis of Nterminal-pro-BNP (NT-pro-BNP). Results Peak oxygen uptake and constant work exercise time (CWET) were significantly lower in COPD group than in control group (15.81±3.65 vs 19.19±6.16 ml/min kg, P=0.035 and 7.78±6.53 min vs 14.77±7.33 rain, P=0.015, respectively). Anaerobic threshold, oxygen pulse and heart rate reserve were not statistically significant between COPD group and control group. The NT-pro-BNP levels both at rest and during constant work exercise were higher in COPD group compared to control group, but without statistical significance. The correlations between CWET and NT-proBNP at rest or during exercise in patients with COPD were not statistically significant. Conclusions Heart failure does not contribute to exercise intolerance in mild-to-moderate COPD.(J Geriatr Cardioi 2009; 6:147-150). 展开更多
