Methods: By using Fos--oncoprotein antibody immunohistochemical technique(ABC method), the distribution of c--fos oncogene expression in the diencophalon was observed under optical microscope (Olympus ). Results: Fifteen Fos--like immunoreactive neurons appeared in the diencophalon. They were Paraventricular thalanlic nucleus. central medial thalarnic nucleus, reuniens thalamic nucleus, rhomboid thalamic nucleus, dorsal endopiriform nucleus. dorsal lateralgeniculate nucleus. lateral posterior thalamic nucleus, laterdorsal thalamic nucleus, periventeriicular hypothealamic nucleus, median preoptic nucleus, supraoptic nucleus, arcute hyothalamic nucleus, suprachiasmatic nucleus, ventral posterolateral thalamic nucleus and habenular nucleus. Conclusion: The inhalationanesthetics in diencophalon do not spread all over the place but have clear acting positions.展开更多

文摘目的 探讨超声引导双侧腹横肌平面(TAP)阻滞复合小剂量瑞芬太尼麻醉诱导在全麻剖宫产手术中的应用效果。方法 选取择期行全麻剖宫产术的产妇90例,随机分为TAP阻滞组(A组)、TAP阻滞复合小剂量瑞芬太尼组(B组)、TAP阻滞复合中剂量瑞芬太尼组(C组)各30例。产妇均在麻醉诱导前30 min行超声引导下双侧腋中线水平TAP阻滞,麻醉诱导时B组推注瑞芬太尼0.25μg/kg、C组推注瑞芬太尼0.5μg/kg、A组推注等体积生理盐水。分别于麻醉诱导前(T0)、切皮即刻(T1)、胎儿取出即刻(T2)记录产妇收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、心率(HR);记录产妇术后24 h内镇痛泵按压次数及术后6、12、24 h的疼痛视觉模拟(VAS)评分;取胎儿脐动脉血进行血气分析,指标包括酸碱度(pH)、血氧分压(PO_(2))、二氧化碳分压(PCO_(2))、乳酸(Lac);记录新生儿出生后1、5 min Apgar评分,记录需要触觉刺激、面罩加压给氧、气管插管的新生儿例数。结果与同组T0时点比较,B组、C组T1、T2时点SBP、DBP、MAP、HR均下降;与同组T1时点比较,C组T2时点SBP、DBP、MAP、HR均下降;与A组同时点比较,B组、C组T1、T2时点SBP、DBP、MAP、HR均下降;与B组同时点比较,C组T1、T2时点SBP、DBP、MAP、HR均下降(P均<0.05)。各组产妇术后6、12、24 h VAS评分及24 h镇痛泵按压次数比较差异均无统计学意义,各组新生儿脐动脉血气分析指标pH、PCO_(2)、PO_(2)、Lac比较差异均无统计学意义(P均>0.05)。C组出生1 min Apgar评分<8分、需要触觉刺激、需要面罩通气的新生儿例数高于A、B组(P均<0.05),A、B组各指标比较差异均无统计学意义。结论 超声引导下双侧腹横肌平面阻滞技术复合0.25μg/kg瑞芬太尼在全麻剖宫产手术中效果较好,且应用安全。
