On May 12, 2008, an 8.0-magnitude earthquake occurred in Wenchuan, Sichuan Province. In this disaster, 69,000 people were killed, 18,000 people were reported missing, and 37,000 people were injured, including more than 10,000 who were seriously injured. Trauma was the most commonly observed type of injury, with fractures accounting for 74% of all injury cases. On April 14, 2010, a 7.1-magnitude earthquake occurred in Yushu, in Qinghai Province. In this disaster, 2,698 people were killed, 270 people were reported missing, and 11,000 people were injured, including more than 3,100 who were seriously injured. Fracture injury accounted for 58.4% of all injury cases. After each earthquake, the Chinese Army Medical Services responded promptly, according to the previously established guidelines, and sent out elite forces to the disaster areas, with the objectives of organizing, coordinating and participating in an efficient and evidence-based medical rescue effort. After the Wenchuan earthquake, 397 mobile medical service teams including 7,061 health workers were sent to the disaster areas. A total of 69,000 casualties were treated, and 22,000 surgeries were performed. After the Yushu earthquake, 25 mobile medical service teams involving 2,025 health workers were sent. They performed 1,635 surgeries and created an astounding outcome of "zero deaths" in the aftermath of the earthquake during their treatment of casualties in a high-altitude region. Within a week after each earthquake, the military teams rescued approximately 60% of the total number of rescued casualties and evacuated approximately 80% of the total number of evacuated sick or wounded victims, playing a critical role and making invaluable contributions to earthquake relief. The experience and lessons learned from the rescue efforts of the Chinese military after the two earthquakes have highlighted several key aspects in emergency medical rescue: 1) medical rescue theories must be updated; 2) military-civilian cooperation must be stressed; 3) professional rescue forces must be strengthened; 4) supporting facilities must be improved; and 5) international exchanges and cooperation must be deepened.展开更多

METHODS: Data from cross-sectional studies focusing on the prevalence of PTSD after earthquakes among the elderly were collected from Pub Med, EMBASE, Cochrane Library, and China National Knowledge Infrastructure in December 2019. The search terms included post-traumatic stress disorder, earthquake, and elderly. This study used Review Manager 5.0 to evaluate the impact of the results. In addition, forest plots, sensitivity analysis, and bias analysis were carried out on the included articles. The combined estimate of the risk ratio and the standard deviation of the 95% confidence interval(95% CI) were measurements of the size of the effect.RESULTS: There were 4,834 patients included from 10 eligible studies. The sample sizes of PTSD group and non-PTSD group were 1,277 and 3,557, respectively. The meta-analysis showed that the overall occurrence of PTSD after earthquakes among the elderly was 0.25;the occurrence in females was higher than that in males, and the occurrence in the same province indicated little difference(Wenchuan city 0.25 and Ya'an city 0.24).CONCLUSIONS: After earthquakes, the occurrence of PTSD is higher among the elderly than among other age groups, and higher among the females than among the males, while there is little difference among different areas within the same province. This indicated that prioritized specific psychological interventions should be provided to the aged and the females.展开更多

The purpose of this study was to summarize people's experiences that how the patients were tracked in past earthquake disasters in Iran.METHODS: A qualitative study was carried out in 2015. This was an interview-based qualitative study using content analysis. The interviewed people included physicians, nurses, emergency medical technicians, disaster managers, Red Crescent Society' first responders and managers. Participants were identified using a snow ball sampling method. Interviews were audiotaped, transcribed, coded, and entered into MAXQDA(version 10) for coding and content analysis.RESULTS: Three main themes and seven categories including content(recoding data), function(identification of victims, identification of the deceased, informing the patients' relatives, patients' evacuation and transfer, and statistical reporting), technology(the state of using technology) were identifi ed that showed the patient tracking status in past earthquakes in Iran.CONCLUSION: Participants believed that to identify and register the data related to patients or the dead, no consistent action plan was available. So developing a consistent patient tracking system could overcome this issue and improve patient safety.展开更多

文摘BACKGROUND:There is limited evidence for emergency physicians and emergency trauma surgeons regarding the determinants of early and rapid assessment of older adult in-hospital mortality due to earthquakes. This study explored factors related to the early and rapid assessment of the mortality among older adult earthquake trauma patients(OAETPs)and created a screening model.METHODS:Data on 7,308 OAETPs from the West China Earthquake Patients Database were analyzed retrospectively. The 35 variables that can be obtained rapidly on arrival at the hospital were collected.Least absolute shrinkage and selection operator(LASSO)regression analysis was performed. Then, the nomogram for assessing the mortality of OAETPs was constructed.RESULTS:We identified 10 independent mortality-related factors that contributed to the in-hospital mortality of OAETPs.The 10 factors included age(odds ratio[OR]=1.061,95%confidence interval[CI]:1.031-1.090),dementia(OR=5.146,95%CI:1.169-17.856),coronary heart disease(CHD;OR=23.441,95%CI:4.799-83.927),malignant tumor(OR=8.497,95%CI:3.583-17.967),deep vein thrombosis(DVT;OR=7.110,95%CI:1.369-27.168),chronic kidney disease(CKD;OR=11.783,95%CI:5.419-24.407),pulse rate(PR;OR=1.036,95%CI:1.022-1.048),mean artery pressure(MAP;OR=0.960,95%CI:0.945-0.975),Glasgow Coma Scale(GCS;OR=0.864,95%CI:0.760-0.972),and Triage Revised Trauma Score(T-RTS,OR=0.485,95%CI:0.351-0.696).CONCLUSION:The 10 mortality-related factors could be quickly obtained on hospital arrival and should be the focal point of future earthquake response strategies regarding hospitalized older adults with trauma.A nomogram was constructed based on the factors for screening OAETPs with a higher risk of in-hospital mortality.
文摘BACKGROUND: The study aims to investigate the occurrence of post-traumatic stress disorder(PTSD) after earthquakes among the elderly. This indicated that prioritized specific psychological interventions should be provided to the aged and the females.

文摘With the rapid development of high-speed railways around the globe, the safety of vehicles running on bridges during earthquakes has been paid more attention to.In the design of railway bridges,in addition to ensuring the safety of the bridge structure in earthquake, the vehicle safety should also be ensured.Previous studies have focused on the detailed analysis of vehicle derailment on bridges, proposing complex numerical algorithms for wheel-rail contact analysis as well as for parametric analysis, but they are inconvenient for designers.Intensity measure(IM)used in performance-based earthquake engineering is introduced in this study.A method to evaluate the vehicle safety on bridges under earthquakes is proposed with respect to the optimal IM.Then, the vehicle derailment case of the Kumamoto earthquake in Japan verifies the decoupling method of vehicle-bridge interaction model. In the assessment of vehicle derailments, eight IMs are systematically compared:the IMs of bridge deck motion are generally better than those of ground motion;the variation coefficient of spectral intensity of the bridge deck is the smallest at different frequencies. Finally, the derailment fragility cloud map is presented to evaluate the vehicle safety on bridges during earthquakes.

文摘BACKGROUND: After a disaster, all victims have to be rapidly and accurately identified for locating, tracking and regulating them. So developing a consistent patient tracking system could overcome this issue and improve patient safety.

文摘There has been much interest in studying quasi-periodic events on earthquake models. Here we investigate quasiperiodic events in the avalanche time series on structured earthquake models by the analysis of the autocorrelation function and the fast Fourier transform. For random spatial earthquake models, quasi-periodic events are robust and we obtain a simple rule for a period that is proportional to the choice of unit time and the dissipation of the system. Moreover, computer simulations validate this rule for two-dimensional lattice models and cycle graphs, but our simulation results also show that small-world models, scale-free models, and random rule graphs do not have periodic phenomena.Although the periodicity of avalanche does not depend on the criticality of the system or the average degree of the system or the size of the system, there is evidence that it depends on the time series of the average force of the system.
文摘How to improve the rescue efficiency of the network after the earthquake is the key content of emergency management decision-making, improve the efficiency of emergency rescue, and reduce the impact of emergency rescue to the non victims. Using cyberspace of lifeline network traffic and emergency transportation problem, considering the network restoration problem of disaster area, earthquake emergency supplies distribution model is established. In the model, to consider the need to repair the damaged sections and the existing emergency rescue generated traffic volume of emergency rescue network effects. And design heuristic algorithm for solving the model. Finally the example shows that in emergency rescue, emergency rescue of critical damage repair the road and traffic control of the whole lifeline network rescue efficiency highest, with the average nuisance greatly reduce, the lifeline network connectivity reliability.

文摘A multi-block model and a corresponding computer program have been developed which predict the kinematics of landslides.Furthermore,a unique event for studying different models simulating the triggering and movement of landslides is the 2008Wenchuan earthquake in the mountainous region in Sichuan Province of China, which caused a large number of rapid landslides. The purpose of the paper is two-fold:(a)to propose and incorporate into the multi-block model constitutive relations predicting soil response along slip surfaces, and(b)to apply the multi-block model with the constitutive relations at landslides triggered by the Wenchuan earthquake. The proposed constitutive equations predict the shape of the shear stress-displacement response measured in ring shear tests. In the application, four landslides caused by the Wenchuan earthquake were considered.Only in one of these landslides the shear resistance-displacement response along the slip surface has been measured in laboratory tests. At this landslide, the triggering and movement of the landslide was predicted. In the other landslides, back analyses were performed and it was observed that the multi-block model predicted reasonably well the final configuration of all slides. In addition,as the measured and back-estimated total friction angle of all landslides was less than 180,and the materials along the slip surface were sandy,it is inferred that some,or all of the slip surface during these slides was sheared in an undrained manner and excess pore pressures generated during sliding played a key role in the triggering and movement of these landslides.Concluding, the paper(A)proposed and validated a multi-block constitutive model which can be applied to predict the triggering and movement of earthquake-induced slides and(B)by analyzing four landslides of the 2008 Wenchuan earthquake,it concludes that some,or all of the slip surface during these slides, was sheared in an undrained manner and excess pore pressures generated during sliding played a key role in the triggering and movement of these landslides.

文摘The frequency-size distribution of model earthquakes in a one-dimensional Burridge-Knopoff spring-block model[Phys.Rev.Lett,22(1989)2632]is studied.Analytical approach is taken to avoid the limitations on the size of models and computation time in numerical calculations.By using the Hamiltonian of the spring-block system a concept of‘seismic phonon’is proposed which is similar to the concept of phonon in solid state physics. The result of the occurrence frequency of model earthquakes shows a distribution similar to the modified gamma distribution proposed by Main and Burton using information theory.

文摘Taking a concrete frame supporting space structure as the research object,we systematically studied its lateral-torsion coupling effect of reverse problems in consistent earthquake excitation.Firstly, based on its reverse forms and features,we put forward a mechanical analysis model(flexibility layer model) and a calculation method using the response spectrum method and the weighted average method, and verified their validity and feasibility using case analysis. The result shows that the translation displacement change trend of the space structure is basically the same whether reverse exists in the supporting structure or not,but the supporting structure torsion has an effect on the displacement with a relative increase of 10%.

文摘Amorphous solids exhibit scale-free avalanches, even under small external loading, and thus can work as suitable systems to study critical behavior and universality classes. The abundance of scale-free avalanches in the entire elastic tension regime of bulk metallic glass(BMG)samples has been experimentally observed using acoustic emission(AE)measurements. In this work,we compare the statistics of avalanches with those of earthquakes, and find that they both follow the Gutenberg–Richter law in the statistics of energies and Omori’s law of aftershock rates, and share the same characteristics in the distribution of recurrence times. These resemblances encourage us to propose the term“glass-quake”to describe avalanches in elastically loaded BMGs.Furthermore, our work echoes the potential universality of critical behavior in disordered physical systems from atomic to planetary scales, and motivates the use of elastic loaded BMGs as valuable laboratory simulators of seismic dynamics.
