摘要
美国甲状腺协会发布的《成人分化型甲状腺癌诊治指南(2025版)》基于最新循证医学证据,对甲状腺癌的诊断、治疗及随访策略进行了系统性更新。该指南融合内科、外科、影像学、病理学等多学科视角,并首次纳入患者代表意见,内容全面覆盖分化型甲状腺癌的全程管理,显著提升了其临床适用性。针对“主动监测”这一热点问题,新版指南在严格评估现有证据与多学科意见的基础上,提出了明确的推荐意见。本文结合日本、韩国相关指南,对该版指南中主动监测部分的关键更新与临床实践要点进行梳理与探讨,以期更好地指导主动监测在甲状腺癌管理中的应用。
The 2025 American Thyroid Association Guidelines for the Management of Differentiated Thy⁃roid Cancer in Adults incorporate the latest evidence⁃based medical research to systematically update the strate⁃gies for diagnosis,treatment,and follow⁃up of thyroid cancer.Integrating multidisciplinary perspectives from in⁃ternal medicine,surgery,radiology,pathology,and other fields—and for the first time including input from pa⁃tient representatives—the guidelines comprehensively cover the entire management process of differentiated thy⁃roid cancer,significantly enhancing their clinical applicability.Regarding the current focus on active surveil⁃lance,the new edition provides clear recommendations based on a rigorous evaluation of existing evidence and multidisciplinary input.This article reviews and discusses key updates and practical aspects of the active sur⁃veillance section in the guidelines,with reference to relevant Japanese and Korean guidelines,with the aim of improving the application of active surveillance in thyroid cancer management.
