摘要
目的:对上尿路结石病人自我管理干预研究进行范围综述,明确干预要素,为临床实践和未来相关研究提供参考。方法:以澳大利亚乔安娜布里格斯研究所(JBI)范围综述指南为方法学框架,检索中英文数据库中相关研究。检索时限为建库至2024年8月31日。对纳入文献进行汇总和分析。结果:纳入13篇文献,其中5项研究分别以奥瑞姆自理理论、PDCA(计划、实施、检查、行动)管理、格林模式、个案管理、信息‐动机‐行为技巧(IMB)模型为理论基础制定干预措施;主要干预方法为以护士为主导的自我管理模式、健康宣教手册、视频指导、应用智能技术等;干预时长多为1~6个月,结局指标包括结石复发率、自我管理能力、24 h尿量等。通过自我管理干预能提升病人的自我管理能力,有效预防结石复发和促进康复。结论:上尿路结石病人自我管理干预研究已初步证实其在改善病人健康行为、预防结石复发方面的积极作用。未来的研究应充分利用多学科干预模式,深入探索病人需求,持续优化干预策略,从而促进病人的长期健康管理与结局改善。
Objective:To conduct a scoping review of self‐management intervention studies for patients with upper urinary tract calculi,clarify the intervention elements,and provide a reference for clinical practice and future related research.Methods:The Joanna Briggs Institute(JBI)Scoping Review Guidelines of Australia were used as the methodological framework to retrieve relevant studies in Chinese and English databases.The search period was from the establishment of the database to August 31,2024.The included literature was summarized and analyzed.Results:Thirteen articles were included,of which five studies used Orem's self‐care theory,Plan‐Do‐Check‐Act(PDCA)management,Greene's model,case management,and Information‐Motivation‐Behavioral Skills(IMB)Model as theoretical foundations to formulate intervention measures.The main intervention methods were video guidance,health education manuals,personalized care,and application of intelligent technology.The intervention duration was mostly 1 to 6 months,and the outcome indicators included stone recurrence rate,self‐management ability,and 24‐hour urine volume.Self‐management intervention can improve patients'self‐management ability,effectively prevent calculi recurrence,and promote recovery.Conclusions:The study of self‐management intervention for patients with upper urinary tract calculi has preliminarily confirmed its positive role in improving patients'health behaviors and preventing stone recurrence.Future research should make full use of multidisciplinary intervention models,deeply explore patients'needs,and continuously optimize intervention strategies to promote patients'long‐term health management and outcome improvement.
