摘要
目的比较不同材料种植牙在口腔修复中对患者牙周环境及咀嚼功能的影响。方法选取2022年1月至2024年1月周口市疾病预防控制中心专科病医院收治的105例种植牙修复的患者,按随机数字表法分为钛合金组(52例,使用钛合金种植体)和纯钛组(53例,使用纯钛种植体)。在治疗后1个月及治疗后3个月时对患者的牙菌斑指数、牙周袋深度情况进行评估,评价患者的咀嚼效能、固位力及满意度。记录炎症及不良反应发生情况。结果治疗后(1个月、3个月)钛合金组牙菌斑指数、牙周袋深度均低于纯钛组(P<0.05);咀嚼效能、固位力高于纯钛组(P<0.05);美观效果、舒适度及语言功能评分高于纯钛组(P<0.05);肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)和干扰素-γ(IFN-γ)水平低于纯钛组(P<0.05),且纯钛组治疗1个月后、3个月后的TNF-α、IL-8和IFN-γ水平均较治疗前升高(P<0.05),但钛合金组较治疗前差异无统计学意义(P>0.05)。钛合金组种植体周围炎的发生率为5.77%(3例),纯钛组发生率为26.42%(14例),钛合金组低于纯钛组(P<0.05),但组间发生刺痛感、桩核脱落、出血、炎症、下唇麻木的发生率比较差异无统计学意义(P>0.05)。结论钛合金种植体在改善牙周环境、咀嚼功能、患者满意度和种植体周围炎发生率方面优于纯钛种植体,炎症反应较轻,临床效果更佳,适合推广。
Objective To compare the effects of dental implants made of different materials on the periodontal environment and masticatory function in patients undergoing oral restoration.Methods A total of 105 patients who received dental implants at Specialty Hospital,Zhoukou Center for Disease Control and Prevention from January 2022 to January 2024 were randomly divided into the titanium alloy group(52 cases,using titanium alloy implants)and the pure titanium group(53 cases,using pure titanium implants).The plaque index and periodontal pocket depth were evaluated at 1 month and 3 months after treatment.Masticatory efficiency,retention force,patient satisfaction,and inflammatory conditions were assessed,along with the incidence of adverse reactions.Results After treatment(1 month and 3 months),the titanium alloy group showed lower plaque index and periodontal pocket depth compared to the pure titanium group(P<0.05).The titanium alloy group demonstrated superior chewing efficiency,retention strength,aesthetic outcomes,comfort,and speech function scores compared to the pure titanium group(P<0.05).Notably,the titanium alloy group exhibited lower levels of tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),and interferon-γ(IFN-γ)than the pure titanium group(P<0.05).However,TNF-α,IL-8,and IFN-γlevels in the pure titanium group increased significantly after both 1-month and 3-month treatments(P<0.05),while no statistically significant changes were observed in the titanium alloy group(P>0.05).The incidence of periapical periodontitis was 5.77%(3 cases)in the titanium alloy group versus 26.42%(14 cases)in the pure titanium group,indicating a lower risk in titanium alloy patients(P<0.05).No statistically significant differences were found between groups regarding occurrences of paresthesia,core dislodgement,bleeding,inflammation,or lower lip numbness(P>0.05).Conclusion Titanium alloy implants are better than pure titanium implants in improving periodontal environment,masticatory function and satisfaction,with mild inflammatory response and better clinical effect,which are suitable for promotion.
