摘要
本文总结1例胃癌术后直肠转移伴癌性肠梗阻患者的全程营养管理。患者入院后,护理团队立即开展全面的护理评估并完善相关检查。患者入科时临床诊断为胃癌术后直肠转移及癌性肠梗阻、重度营养不良;护理问题为营养失调、潜在并发症-血栓。基于其病情特点及营养风险,制定并实施了肠内外联合营养支持方案,同时加强输液港规范化护理及全程功能锻炼指导与健康教育。经过多学科团队的积极治疗与精细化护理,患者病情显著改善并顺利出院。为期3个月的电话随访结果显示患者血糖控制平稳,营养状况显著改善,整体健康状况良好。
This article summarizes the whole process of nutritional management of a patient with rectal metastasis and cancerous intestinal obstruction after gastric cancer surgery.The nurs⁃ing team had carried out a comprehensive evaluation on illness condtion and protential risks in nursing of the patient.At the time of admission,the patient was clinically diagnosed with rectal metastasis,cancerous intestinal obstruction,and severe malnutrition after gastric cancer surgery.Nursing problems included nutritional disorders,potential complications-thrombosis,etc.Key points of whole process nursing management were dynamic nutritional assessment,parenteral and extra intestinal nutritional support,infusion port care,and health education for the whole process of functional exercise.After active treatment and meticulous care,the patient's condition im⁃proved and he was discharged from the hospital.After 3 months of telephone follow-up,the pa⁃tient’s blood glucose control was stable,the nutritional status was improved,and the overall con⁃dition was good.
