重复经颅磁刺激治疗带状疱疹后神经痛的研究进展

摘要 带状疱疹后神经痛(PHN)是带状疱疹常见的慢性并发症之一,以持续性疼痛为主要特征,严重影响患者生活质量。重复经颅磁刺激(rTMS)作为一种非侵入性神经调控技术,近年来在PHN治疗中展现出潜在应用价值。该文从基础研究及临床研究两方面综述rTMS治疗PHN的研究进展。基础研究表明,rTMS可减少脊髓背角神经元的异常放电,调节外周神经兴奋性影响神经递质释放,进而通过调节大脑皮层兴奋性、抑制疼痛相关神经环路及促进内源性镇痛物质释放等途径发挥镇痛作用。临床研究显示,rTMS安全性较高,但镇痛效果维持时间会受个体差异、刺激参数影响,临床需根据患者情况合理选择刺激强度、部位及时间。 Postherpetic neuralgia(PHN)is one of the common chronic complications of herpes zoster,mainly characterized by persistent pain,which seriously affects the quality of life of patients.Repetitive transcranial magnetic stimulation(rTMS),as a non-invasive neuroregulation technique,has shown potential application value in the treatment of PHN in recent years.This article reviews the research progress of rTMS in the treatment of PHN from both basic research and clinical research aspects.Basic research indicates that rTMS can reduce abnormal discharges of spinal dorsal horn neurons,regulate peripheral nerve excitability to affect the release of neurotransmitters,and thereby exert analgesic effects through pathways such as regulating cerebral cortex excitability,inhibiting pain-related neural circuits,and promoting the release of endogenous analgesic substances.Clinical studies have shown that rTMS has a relatively high safety level,but the duration of analgesic effect can be affected by individual differences and stimulation parameters.Clinically,the intensity,location and duration of stimulation should be reasonably selected based on the patient's condition.
