摘要
目的 分析对比Salter与Pemberton骨盆截骨术对发育性髋关节发育不良患儿髋关节功能及并发症发生率的影响。方法 回顾性选取2020年12月至2023年12月南阳市中医院收治的70例髋关节发育不良患儿作为研究对象,依据手术方法分为研究组(39例)、对照组(31例),前者接受Pemberton骨盆截骨术,后者接受Salter骨盆截骨术。比较两组临床疗效、手术前后影像学指标(中心边缘角、骨性髋臼指数、髋臼指数)、下肢运动功能、髋关节功能、并发症。结果 术后,两组总有效率相比,差异无统计学意义(P>0.05);术后16周,与对照组相比,研究组中心边缘角指标较高,骨性髋臼指数、髋臼指数指标较低(P<0.05);术后16周,与对照组相比,研究组下肢运动功能、髋关节功能评分较高(P<0.05);两组并发症发生率相比,差异无统计学意义(P>0.05)。结论 Pemberton骨盆截骨术与Salter骨盆截骨术均能获得良好疗效,与Salter骨盆截骨术相比,Pemberton骨盆截骨术能改善髋关节影像学指标,促进髋关节功能、下肢运动功能恢复,安全性较好。
Objective To analyze and compare the effect of Salter and Pemberton pelvic osteotomy on hip function and incidence of complications in children with developmental hip dislocation.Methods A total of 70 children with hip dislocation treated in Nanyang Hospital of Traditional Chinese Medicine from December 2020 to December 2023 were selected as the research objects,and they were divided into a study group(39 cases)and a control group(31 cases)according to surgical methods.The former underwent Pemberton pelvic osteotomy and the latter underwent Salter pelvic osteotomy.The clinical efficacy,imaging indexes(central edge angle,bony acetabular index,acetabular index),lower limb motor function,hip joint function before and after operation,and complications were compared between the two groups.Results There was no significant difference in the total effective rate between the two groups(P>0.05).At 16 weeks after operation,compared with the control group,thecentral edge angle index in the study group was higher,while the indexes of bony acetabular index and acetabular index were lower(P<0.05).At 16 weeks after operation,the scores of lower limb motor function and hip function in the study group were higher than those in the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Pemberton pelvic osteotomy and Salter pelvic osteotomy can obtain good curative effect.Compared with Salter pelvic osteotomy,Pemberton pelvic osteotomy can improve the imaging indicators of hip joint,promote the recovery of hip joint function and lower limb motor function,and has good safety.
